Abstract
Background
Lymphoepithelioma-like carcinoma (LELC) is an undifferentiated epithelial tumour with a prominent lymphoid infiltrate, originally described within the nasopharynx. Tumours with a similar histological pattern have been reported in many sites under the term of “lymphoepithelioma-like carcinoma”, including the urinary tract, mainly the bladder and rarely the ureter.
Case presentation
An 81-year-old male patient, who presented with painless gross haematuria was admitted to the urological department. A computed tomography scan of the abdomen and pelvis suggested a localized ureteral tumour with dilatation of the upper pyelocaliceal system. A right nephroureterectomy was performed.
Results
Upon opening the ureter, a solitary tumour measuring 2 cm was identified protruding into the lumen of the upper portion of the ureter, approximately 12 cm from the distal margin of surgical resection. Microscopic examination revealed a diffusely infiltrative process composed of syncytium-like sheets of large undifferentiated cells, with vesicular nuclei, prominent nucleoli and a high mitotic activity, intermingled with a dense lymphoid infiltrate. An in situ urothelial carcinoma was identified in the overlying urothelium.
Immunohistochemically, the tumour cells were strongly positive for cytokeratins AE1/AE3 and CK7, EMA, p63, but unstained by CK20. Lymphoid markers demonstrated diffuse expression of CD45 in the lymphocytic infiltrate, and both T cells (CD3) and B cells (CD20) were present with a predominance of T cells. The neoplastic cells were negative for EBV by both immunohistochemistry and in situ hybridization. These findings were consistent with the diagnosis of lymphoepithelioma-like carcinoma. Seven days after surgery, the patient suffered profuse gastrorrhagia. Regrettably, he died two days later from severe anaemia and multiple organ failure.
Conclusion
Recognizing the lymphoepithelioma-like carcinoma in its pure form, or as a component of urothelial carcinoma appears to be very important. Its identification has diagnostic and therapeutic implications. Differentiating this lesion from malignant lymphoma and chronic infection is important. All the reported cases with ureteral LELC have been successfully treated with surgical intervention alone.

Το λεμφοεπιθηλιόμορφο καρκίνωμα είναι νεόπλασμα με μορφολογικά χαρακτηριστικά παρόμοια με εκείνα του αδιαφοροποίητου ρινοφαρυγγικού καρκινώματος, το όποιο αναπτύσσεται εκτός ρινοφάρυγγα. Το λεμφοεπιθηλιομορφο καρκίνωμα του ουρητήρα είναι εξαιρετικά σπάνιο, με μόνο 10 αναφερόμενες περιπτώσεις στη διεθνή βιβλιογραφία.
Περιστατικό
Aφορα άρρεν, ηλικίας 81 ετών, ο οποίος προσήλθε στο ουρολογικό ιατρείο εξαιτίας ανώδυνης μακροσκοπικής αιματουρίας. Στην αξονική τομογραφία της κοιλιακής και πυελικής περιοχής παρατηρήθηκε εντοπισμένος ουρητηρικος όγκος ο οποίος συνοδεύονταν από διάταση του υπερκείμενου πυελοκαλικικού συστήματος. Βάσει της κλινικής εικόνας ουρητηρικού νεοπλάσματος, ο ασθενής υποβλήθηκε σε δεξιά νεφροουρητηρεκτομη για διαγνωστικούς και θεραπευτικούς σκοπούς.
Αποτελέσματα
Kατα τη διάνοιξη του ουρητήρα διαπιστώθηκε μονήρες νεόπλασμα, μ.δ 2εκ., το όποιο προέβαλλε στον άυλο του, σε απόσταση 12εκ. από το άπω χειρουργικό όριο εκτομής.
Η ιστοπαθολογικη εξέταση ανέδειξε την παρουσία νεοπλάσματος αποτελούμενου από ευμεγέθη αναπλαστικά κύτταρα σε συγκυτιακή διάταξη, με ασαφή περιφερειακά όρια, μεγάλους ασκοειδείς πυρήνες, εμφανή πυρήνια και αρκετές μιτώσεις. Το νεόπλασμα συνοδεύονταν από πυκνές λέμφο-πλασματοκυτταρικες φλεγμονώδεις διηθήσεις του στρώματος. Ανοσοϊστοχημικά, τα νεοπλάσματικα κύτταρα εκφράζονταν θετικά στις κερατινες ΑΕ1/ΑΕ3, στην CK7, στην ΕΜΑ, p63 και αρνητικά στην κερατίνη CK20. Oι λεμφικοί δείκτες έδειξαν διάχυτη έκφραση του CD45, με την παρουσία τόσο των Β κυττάρων (CD20) όσο και των Τ-κυττάρων (CD3). Τα νεοπλασματικα κύτταρα εκφράζονταν αρνητικά στον ιο του EBV τόσο ανοσοιστοχημικα όσο και σε Υβριδισμό in situ. Tα παραπάνω ευρήματα είναι συμβατά με διάγνωση λεμφοεπιθηλιομορφου καρκινώματος του ουρητήρα.
Συμπερασματικά
To λεμφοεπιθηλιομορφο καρκίνωμα του ουρητήρα είναι μια σπάνια νεοπλασματική οντότητα που εκδηλώνεται με αιματουρία. Η αναγνώριση της νόσου έχει μεγάλη διαγνωστική και θεραπευτική σημασία. Η διαφορική διάγνωση από ένα λέμφωμα ή χρόνια αντιδραστική φλεγμονή είναι ουσιαστικής σημασίας. Η συμβολή της ανοσοϊστοχημείας στην περίπτωση αυτή είναι τεράστια.
Η θεραπεία επιλογής σε όλες τις περιπτώσεις ήταν η χειρουργική αφαίρεση του νεοπλάσματος. Όλες οι περιπτώσεις, εκτός της δικής μας, είχαν καλή μετεγχειρητική πορεία. Στην περίπτωση μας, κατά την έβδομη μετεγχειρητική ημέρα, ο ασθενής υπέστη γαστρορραγία και παρόλες τις θεραπευτικές παρεμβάσεις στην μονάδα εντατικής θεραπείας, δυο ημέρες αργότερα, υπέκυψε από σοβαρή αναιμία.
