Hintergrund
Jeder Operateur, der sich mit funktioneller und ästhetischer Nasenchirurgie beschäftigt, wird früher oder später mit dem Thema der Revisionschirurgie konfrontiert. Dabei spielt der ästhetische Anspruch der Patienten eine große Rolle. Oft sind es kleine Details, die diese unzufrieden machen.
Fragestellung
Die Revisionschirurgie der Nase kann über einen offenen oder über geschlossene Zugänge erfolgen. Welche Methode ist für eine Revisionsoperation die jeweils beste und welches sind die wichtigsten Indikationen sowie der richtige Zeitpunkt für die Revision?
Methode
Der Beitrag diskutiert Indikationen, Zeitpunkt und Technik der Revisionsoperationen. Er verdeutlicht an Fallbeispielen die Vorteile und Grenzen endonasaler, auch endoskopisch kontrollierter Operationsschritte.
Schlussfolgerungen
Mithilfe eines Rankings der störenden Merkmale kann der Operateur entscheiden, ob er mit kleinen, minimal-invasiven Operationsschritten über geschlossene Zugänge oder einen offenen Zugang die Hauptprobleme lösen kann. Es stehen dabei die Prinzipien der Camouflage mit gezielten Abtragungen, Glättungen und Augmentationen dem der statischen Rekonstruktion gegenüber. Endoskope kompensieren die Nachteile der geschlossenen Technik durch eine Visualisierung in der Tiefe des Operationsfelds. Miniaturisierte Instrumente sind bei Revisionen des Nasenrückens, des Septums und der Klappenregion hilfreich. Das Prinzip minimaler Invasivität mit effektiver Zielrichtung auf die Korrektur einzelner Formstörungen kann erneute Risiken vermeiden.

The surgeon always has various options when planning a revision procedure of the nose. It is helpful to have the patient rank specific non-aesthetic features in order of importance. There are often small details with which they are dissatisfied
Objectives
Revision surgery of the nose can be carried out by an open or closed approach. Which of these methods is the best for a revision operation, what are the most important indicators and the best time point for a revision?
Methods
This article discusses the indications, time point and technique for a revision operation and using case examples emphasizes the advantages and limits of endonasal and endoscopically guided operation steps.
Conclusion
The priority list provides the basis for deciding whether the main problems can be solved by minimally invasive touch-ups through a closed approach or whether an open approach is required. One option is based on camouflage principles using selective, circumscribed resection and augmentation. Another option is the structural reconstruction via an open approach. Endoscopes can improve endonasal techniques by visualization in the depth of the surgical field. New miniaturized instruments help in procedures on the nasal dorsum, the nasal vault and septum. The principle of minimally invasive approaches with effective targeting for correction of isolated features can avoid new risks.
