Résumé
Introduction
Malgré le caractère antagoniste et potentiellement délétère de la coprescription d’un anticholinestérasique (IChE) et d’un traitement possédant des propriétés anticholinergiques, cette association reste fréquente dans la population fragile et dépendante des résidents en EHPAD. l’objectif de cette étude était d’évaluer l’impact à un mois du signalement aux médecins traitants d’une prescription inappropriée associant IChE/anticholinergique, sur le nombre de prescriptions inappropriées concernant cette association, au sein d’une population de patient vivant en EHPAD.
Méthode
Il s’agissait d’une évaluation des pratiques professionnelles par suivi d’indicateur après intervention auprès des médecins traitants par les médecins coordinateurs.
Résultats
Sur les 1 367 ordonnances étudiées (22 EHPAD sur trois départements), 50 associations inappropriées ont été retrouvées. l’intervention a permis de diminuer de 68 % (p < 0,001) le nombre de prescriptions inappropriées associant IChE/anticholinergique.
Discussion
l’intervention confraternelle du médecin coordinateur auprès du médecin traitant permet une réduction importante et significative du nombre de prescriptions inappropriées en EHPAD.
Abstract
Introduction
Inappropriate association between anticholinergic and anticholinesterase (IChE) drug is frequent in nursing home, although it is known to be potentially deleterious. The objective of this study was to assess after one month the impact of an intervention from the coordinating physician to the general practitioner (GP) concerning inappropriate association between IChE and anticholinergic drug.
Method
The study was an evaluation of professional practice with tracking indicator.
Results
Thousand three hundred sixty-seven prescriptions were analyzed in 22 nursing homes. Fifty inappropriate prescriptions were found and transmitted to the GP. This intervention reduced inappropriate prescriptions from 50 to 16 (68%, P < 0.001).
Discussion
The intervention of the medical coordinator to the general practitioner allows an important and significant reduction in the number of inappropriate drug prescriptions in nursing homes.
