Zusammenfassung
Die potenziellen Symptome einer Refluxkrankheit sind vielfältig und unspezifisch. Eine apparative Diagnostik ist nicht bei jedem Patienten mit Refluxsymptomen zwingend erforderlich, wohl aber bei Vorliegen von Alarmsymptomen, Risikofaktoren, untypischen Symptomen oder bei Versagen einer 4‑wöchigen Standardtherapie. An erster Stelle steht dann die Ösophagogastroduodenoskopie (ÖGD), aber auch Refluxmessungen, auf denen der Schwerpunkt dieses Beitrags liegt, können erforderlich sein. Diese umfassen die konventionelle sondenabhängige pH-Metrie über 24 h, die kabellose pH-Metrie und die kombinierte Impedanz-pH-Metrie sowie die nur noch selten eingesetzte Bilimetrie. Refluxmessungen sind zur Diagnosesicherung vor geplanter Antirefluxoperation sowie zur Diagnosestellung bei endoskopisch-negativen Formen der Refluxkrankheit von besonderer Bedeutung. Sie werden dann in der Regel nach Absetzen der Therapie mit Protonenpumpeninhibitoren (PPI) durchgeführt. Die pH-Metrie und insbesondere die Impedanz-pH-Metrie können aber auch klären, ob ein Therapieversagen auf dem Fortbestehen von saurem Reflux (pH-Metrie, ggf. bipolar) oder auf pathologischem nichtsaurem Reflux (Impedanz-pH-Metrie) beruht. Zu diesem Zweck sollten sie unter Fortführung der PPI-Therapie erfolgen. Die (hochauflösende) Ösophagusmanometrie trägt nicht zur eigentlichen Diagnosestellung bei Refluxerkrankung bei, durch sie können aber Pathomechanismen geklärt und Differenzialdiagnosen aufgezeigt werden.
Abstract
The potential symptoms of reflux disease are varied and nonspecific. Instrument-based diagnostic workup is not imperative for every patient with reflux symptoms, but is necessary in the presence of alarm symptoms, risk factors, atypical symptoms or failure of a 4‑week standard therapy. Esophagogastroduodenoscopy (EGD) and reflux measurements, which are the focus of this article, may be necessary. These include conventional probe-dependent pH monitoring over 24 h, wireless pH monitoring, combined impedance pH monitoring, and the rarely used bilimetry. Reflux measurements to confirm the diagnosis during the work-up for antireflux surgery and the diagnosis of endoscopically negative forms of reflux disease are of particular importance. They are usually performed after discontinuation of therapy with proton pump inhibitors (PPI). However, pH monitoring and in particular impedance pH monitoring can also be useful to determine whether treatment failure is due to the persistence of acid reflux (pH monitoring, bipolar) or pathological nonacid reflux (impedance pH monitoring). For this purpose, they should be performed under continued PPI therapy. (High-resolution) esophageal manometry does not contribute to the actual diagnosis of reflux disease, but it can be used to clarify pathomechanisms and for differential diagnosis.
