Zusammenfassung

Bei einem 4‑jährigen Jungen bestanden seit Monaten eine Atembehinderung und persistierender Husten. Die Thoraxröntgenuntersuchung ergab eine miliäre, teils ringförmige, teils fleckig-granuläre vermehrte Zeichnung der Lunge. Tuberkulintest, Magensaft, Schweißtest, Chymotrypsin im Stuhl und immunreaktives Trypsin im Serum waren unauffällig. In der thorakalen Computertomographie fand sich ein „sandsturmartiges“ Bild mit interlobulär verdickten Septen, diffuser Dichteanhebung und mikronodulären Verkalkungen. Die Lungenbiopsie zeigte eine intraalveoläre lamelläre Mikrolithiasis. Somit wurde eine pulmonale alveoläre Mikrolithiasis festgestellt. Die Diagnosesicherung erfolgt dabei durch Sequenzierung des SLC34A2-Gens.

Abstract

A 4‑year-old boy presented with difficult breathing and cough persisting for several months. The chest x‑ray revealed numerous miliary, partly circular, partly speckled granular opacities in the lungs. The tuberculin test, gastric juices, sweat test, chymotrypsin in stool, and serum immunoreactive trypsin were normal. The thoracic computed tomography showed a “sand storm-like” image with thickened interlobular septa, diffuse increase in density, and micronodular calcifications. Lung biopsy showed intra-alveolar lamellar microliths. Thus, pulmonary alveolar microlithiasis was diagnosed, which was confirmed by sequencing of the SLC34A2 gene.
