Successful re-ablation of a permanent junctional reciprocating tachycardia with cryoenergy

Case report of a 19-year-old patient

Summary

We report the case of a 19-year-old male patient who presented with a permanent junctional reciprocating tachycardia (PJRT). After a primarily successful radiofrequency ablation of a para-Hisian, midseptal, accessory pathway, recurrence of tachycardia was documented. Thereafter, successful ablation using cryoenergy was performed. Since this second ablation the patient has been free of tachycardia. Our case study shows that the treatment of PJRT in young adults using cryoenergy can be successfully and safely conducted, especially after tachycardia recurrence following an initial radiofrequency ablation.

Zusammenfassung

Es wird über den Fall eines 19-jährigen männlichen Patienten berichtet, der sich mit einer permanenten junktionalen Reentry-Tachykardie (PJRT) vorstellte. Nach einer primär erfolgreichen Radiofrequenzablation einer parahisären akzessorischen Leitungsbahn in midseptaler Position wurde ein Wiederauftreten der Tachykardie dokumentiert. Anschließend wurde eine erfolgreiche Ablation mit Kryoenergie durchgeführt. Seit dieser zweiten Ablation war der Patient frei von Tachykardien. Unser Fallbericht zeigt, dass die Behandlung einer PJRT bei jungen Erwachsenen mittels Kryoenergie erfolgreich und sicher durchgeführt werden kann, insbesondere nach Wiederauftreten einer Tachykardie nach initial erfolgter Radiofrequenzablation.
