Zusammenfassung
Hintergrund
Fieber ist ein sehr häufiges Begleitsymptom in der Primärmedizin. Der initiale Zugang in einem präklinischen Setting unterscheidet sich von Diagnostik und Therapie im Krankenhaus und ist daher auch für die Notfallmedizin von Bedeutung.
Ziel
Die Prinzipien der präklinischen und ambulanten Versorgung erwachsener Patienten mit Fieber werden in diesem Beitrag dargestellt.

Recherche und Auswertung der Literatur.
Ergebnisse und Schlussfolgerungen
Eine zugrunde liegende Erkrankung muss und kann nicht immer diagnostiziert werden. Wichtig ist das Erkennen von abwendbar gefährlichen Verläufen, die einer sofortigen Behandlung bedürfen. Anamnese und körperliche Untersuchung sind oft ausreichend, um eine initiale Entscheidung zum therapeutischen Vorgehen treffen zu können. Unter Umständen folgt das weitere Vorgehen einer Stufendiagnostik. Biomarker sind mangels erwiesenen Benefits in der Therapieabwägung allenfalls additiv zur klinischen Wahrscheinlichkeit sinnvoll. Auch die Höhe der Fiebertemperatur kann weder sicher mit der Schwere einer Erkrankung noch mit der Ursache korreliert werden. Einen aussagekräftigeren Prädiktor stellt der allgemeine Zustand des Patienten dar. Eine günstige Nutzen-Schaden-Relation zwischen unerwünschten Arzneimittelwirkungen, Rekonvaleszenzdauer und symptomatischer Besserung durch antipyretische Medikamente existiert bislang nicht. Antipyretika können aber aufgrund ihrer analgetischen Effekte, unter Beachtung von Kontraindikationen, zur Symptomkontrolle eine Option darstellen. Besonders bei älteren Patienten sollte auf Zeichen einer Dehydratation geachtet werden und ein Flüssigkeitsdefizit zur Vermeidung komplizierender Faktoren ausgeglichen werden.
Abstract
Background
Fever is a common symptom in primary care. The initial preclinical diagnostic and therapeutic approach differs from hospital settings and is therefore relevant to emergency medicine.
Objectives
The principles of preclinical outpatient care of adults with feverish illnesses are presented.
Methods
Literature search and analysis.
Results and conclusions
In primary care, an underlying medical condition can sometimes not be defined. More importantly, avoidable life-threatening conditions must be recognized in order to act appropriately. Patient history and physical examination are often sufficient to decide which initial therapeutic steps should be taken. In certain cases, staged diagnostics may be required. The analysis of biomarkers alone has no evident benefit and therefore biomarkers can only add security to clinical probabilities. Moreover, the level of fever cannot be correlated with either the severity of the disease or with the cause. A more significant predictor is the patient’s state. Taking into consideration adverse drug effects, convalescence time, and symptomatic relief, no positive risk–benefit evaluation has been made for antipyretic medicine. However, due to their analgesic effects they can still be an option for symptomatic therapy if no contraindications are present. Especially elderly patients should be monitored for signs of dehydration, and fluid deficits should be compensated via oral intake.
