Zusammenfassung
Die Endo-Exo-Femurprothese („integral leg prosthesis“, ILP) wurde von 1999 bis 2015 bei über 110 Patienten in Deutschland angewendet. Grundprinzip dieser Methode ist zunächst das zementfreie Einbringen einer Stielprothese in den Restknochen oberschenkelamputierter Patienten, wobei die Weichteile danach vorübergehend wieder verschlossen werden. Bei zunehmendem Einwachsen (Osseointegration) des Stiels folgen in einem zweiten Schritt nach ca. 6 bis 8 Wochen das Ausleiten durch die Haut über einen Adapter und der Anschluss an eine Exoprothese. Die Vorteile dieser Technik wie vermehrter Sitzkomfort, verbesserte Propriozeption, erhöhte Mobilisationsfähigkeit, verbesserte Energiebilanz und physiologische Hüftgelenkbelastung werden den systemimmanenten Nachteilen wie mögliche aufsteigende Weichteilinfektion und Weichteilirritation im Stomabereich gegenübergestellt und anhand von Beispielen erläutert. Eine statistische Auswertung dieser individuellen Versorgungsmethode erscheint bei den niedrigen Fallzahlen noch nicht sinnvoll.
Abstract
Between 1999 and 2015 endo-exo femoral prostheses or integral leg prostheses (ILP) have been implanted in more than 110 patients in Germany. The basic principal of these methods for above-knee amputees is the cement-free implantation of a stem prosthesis in the residual femoral stump using the press-fit technique and the soft tissue is subsequently temporarily closed again. As the stem becomes increasingly incorporated (osseointegration) a second step is carried out after 6–8 weeks and a stoma is cut out through the skin to connect the stem to the adapter for the exoprosthesis. The advantages of these techniques, such as more comfort, improved proprioception, increased mobilization, better energy balance and physiological hip joint stress are contrasted with the disadvantages, such as possible ascending infections and soft tissue irritation around the stoma and illustrated by case examples. A statistical evaluation of these individualized and relatively infrequently used methods does not yet seem to be of any value.
