Zusammenfassung
Hintergrund
In den letzten Jahren wurden in vielen Disziplinen der pädiatrischen Neuroonkologie neue Erkenntnisse generiert. Dazu zählen wichtige Fortschritte zum Verständnis der Biologie von Medulloblastom, Ponsgliom, niedrig- und hochgradigem Gliom, Ependymom, embryonalem Tumor mit mehrschichtigen Rosetten (ETMR) und atypischen teratoiden/rhabdoiden Tumoren (AT/RT), die eine neue Ära klinischer und molekularer Diagnostik sowie Therapie eingeläutet haben.
Methoden
Forschungsergebnisse des deutschen Behandlungsnetzwerks HIT sowie eine selektive Literaturübersicht publizierter Studien aus den letzten 40 Jahren sind die Grundlage dieser Arbeit.
Ergebnisse
Moderne Therapiekonzepte werden, basierend auf dem erweiterten Verständnis der Tumorbiologie, schrittweise in klinische Behandlungsempfehlungen integriert. Aktuelle Therapieansätze werden nach klinischem und biologischem Risiko modifiziert, um so die Langzeittoxizität bei Patienten mit guter Prognose zu reduzieren und ungünstige Heilungsraten in Hochrisikogruppen zu verbessern. Neue bildgebende Verfahren wie die Verfügbarkeit von intraoperativem MRT haben neurochirurgische Operationen begünstigt. Verbesserungen in der Radiotherapieplanung und zunehmende Verfügbarkeit der Protonentherapie schüren Hoffnung auf langfristig verminderte Morbidität nach Bestrahlung. Ein besseres Verständnis der Pathophysiologie der Schäden für die Entwicklung des Gehirns und andere therapieassoziierte Spätfolgen rücken zunehmend in den Fokus der Forschung − mit dem Ziel, Schäden zu mildern und den Kindern Bewältigungsmechanismen bieten.
Schlussfolgerungen
Für die Validierung und Weiterentwicklung der aufgezeigten biologischen und klinischen Aspekte sind auch künftig prospektive Studien im Behandlungsnetzwerk HIT erforderlich.
Abstract
Background
In recent years, new insights into many disciplines of pediatric neuro-oncology have been generated. These include important progress in understanding the biology of medulloblastoma, pontine glioma, low and high-grade glioma, ependymoma, embryonal tumor with multilayered rosettes (ETMR) and atypical teratoid/rhabdoid tumor (AT/RT). This ushered in a new era of clinical and molecular diagnostics and therapy.
Methods
This study is based on the research results of the German treatment network HIT as well as a selective literature review of studies published over the last 40 years.
Results
Modern therapy concepts are gradually becoming integrated into clinical treatment recommendations based on the expanded understanding of tumor biology. Current therapeutic approaches are modified according to the clinical and biological risk in order to reduce the long-term toxicity in patients with a good prognosis and to improve the poor cure rates in high-risk groups. Novel imaging techniques, such as the availability of intraoperative magnetic resonance imaging (MRI) have facilitated neurosurgery. Improvements in radiotherapy planning and the increasing availability of proton beam therapy raise hopes for a long-term decrease in morbidity after irradiation. A better understanding of the pathophysiology of damage to the developing brain and other treatment-related complications are increasingly becoming the focus of research, with the aim of mitigating damage and providing children with coping mechanisms.
Conclusions
The validation and further development of the biological and clinical aspects described are required in future prospective studies within the HIT treatment network.
