Zusammenfassung
Die frühe Mukositis der Mundschleimhaut ist eine regelmäßige, Dosis limitierende Nebenwirkung der Strahlentherapie von Kopf-Hals-Tumoren. In der Fachliteratur findet sich eine Vielzahl experimenteller und klinischer Untersuchungen zu Prävention und Behandlung. Jedoch ist die Wirksamkeit für die Mehrzahl der Ansätze klinisch nicht nachgewiesen. In diesem Überblick sollen exemplarisch klinische Maßnahmen und präklinische Ansätze zur Beeinflussung der oralen Mukositis dargestellt werden. Zur Prophylaxe der oralen Mukositis sind neben einer gründlichen Zahnsanierung die Verbesserung und Intensivierung der Mundhygiene sowie häufige Mundspülungen effektiv. Die Bedeutung einer engmaschigen persönlichen Betreuung der Patienten ist zu betonen. Die vorsorgende Anlage eines perkutanen endoskopischen Gastrostoma (PEG) muss am Zustand des Patienten und der zu erwartenden Beeinträchtigung der Nahrungsaufnahme orientiert werden. Als therapeutische Maßnahmen bei manifesten Schleimhautreaktionen kommen zusätzlich eine lokale bzw. systemische Schmerzbehandlung sowie die lokale Applikation von Antimykotika und Antibiotika in Betracht.
Abstract
Early mucositis is a frequent, dose-limiting side effect of radiotherapy of head and neck tumours. A variety of experimental and clinical studies on its prophylaxis and management have been carried out. However, for most approaches, no efficacy has been clinically demonstrated. This review discusses the clinical strategies and preclinical approaches for the amelioration of oral mucositis. In addition to thorough dental restoration, an optimization and intensification of oral hygiene with frequent mouthwashes is effective for the prevention of oral mucositis. Intense supervision of the patient is recommended. The necessity for a percutaneous endoscopic gastrostomy is dependent on the status of the patient and the expected impairment of food uptake. Therapeutic intervention is restricted to local or systemic analgesia and the local application of antimycotics and antibiotics.
