Objektivierbare Gesichtsfeldeinschränkung bei Patienten mit Dermatochalasis vor und nach Blepharoplastik-Operation

Zusammenfassung

Zielsetzung

Die Evaluation funktioneller Blepharoplastikoperationen der Oberlider durch Objektivierung und Vergleich der superioren und temporalen Gesichtsfeldeinschränkung prä- und postoperativ, die Korrelation zwischen subjektiven Patientenangaben und objektivierbaren, postoperativen Gesichtsfeldänderungen sowie die Untersuchung des Einflusses von periorbitalen Strukturen auf das Gesichtsfeld.

Methodik

Die Daten von insgesamt 19 Patienten wurden miteinander verglichen. Alle Patienten wurden prä- sowie postoperativ mit einem Goldmann-Perimeter untersucht, zusätzlich wurden postoperative Fotografien und eine Fragebogenuntersuchung durchgeführt.

Ergebnisse

Die Untersuchung ergab am superioren Gesichtsfeld eine signifikante Verbesserung, wohingegen am temporalen Gesichtsfeld keine signifikante Verbesserung erzielt werden konnte. Die OCH-Winkel zeigten auf der linken Seite einen Mittelwert von 65,1 (± 8,7) Grad und auf der rechten 65,3 (± 9,5) Grad, es konnten aber keine signifikanten Zusammenhänge zwischen dem Winkel und den prä- sowie postoperativen superioren Gesichtsfeldern gefunden werden. Die Patientenangaben waren zum Großteil vom objektivierbaren Ergebnis der Operation unabhängig.

Schlussfolgerungen

Die funktionelle Blepharoplastikoperation der Oberlider zeigte unabhängig vom Geschlecht am superioren Gesichtsfeld eine hohe Wirksamkeit. Für das temporale Gesichtsfeld war jedoch die Wirksamkeit gering.

Summary

Purpose

To evaluate functional upper eyelid blepharoplasty operations regarding the comparison of measurable pre- and postoperative visual field limitations, the relation between subjective patient information and the measurable change in the postoperative field of vision as well as the influence of periorbital structures on the field of vision were investigated.

Methods

19 patients who underwent pre- and postoperative perimetry with a Goldmann-perimeter were compared. Additionally, postoperative pictures were made and each patient answered a standardised questionnaire.

Results

A significant improvement in the superior visual field was found, whereas the temporal visual field only improved nonsignificantly. The mean OCH-angle was 65.1 (± 8.7) degrees on the left and 65.3 (± 9.5) degrees on the right, however, no significant correlations between the angle and the pre- and postoperative superior visual field could be found. The subjective patient information did not depend on the measurable result of the operation in most cases.

Conclusion

Functional upper eyelid blepharoplasty proved to be highly efficient in improving the superior visual field. The effect on the temporal visual field was limited.
