Zusammenfassung
Der Zusammenhang zwischen atypischen Femurfrakturen und Langzeit-Bisphosphonattherapie ist derzeit Grundlage vieler internationaler Forschungsarbeiten und kontroversieller Diskussionen. Die Suppression des Bone Turnovers unter antiresorptiver Therapie mit Bisphosphonaten ist therapeutischer Standard in der Osteoporosebehandlung seit Jahrzehnten. In diesem Fallbericht beschreiben wir retrospektiv die Entwicklung einer kortikalen Stressfraktur im Verlauf von 5 Jahren mit dem Resultat einer atypischen subtrochantären Fraktur bei einer 75-jährigen Patientin mit bekannter postmenopausaler Osteoporose und Bisphosphonattherapie über 15 Jahre. Nach 10 Jahren antiresorptiver Behandlung trat bei der Patientin eine kortikale Stressfraktur im rechten Femur auf. Unter Fortführung der Therapie für weitere 5 Jahre kam es zu einer spontanen atypischen subtrochantären Fraktur. Eine Erklärungsmöglichkeit wird in Mikrofrakturen gesehen, welche infolge der Suppression des physiologischen Knochenumbaus an Lokalisationen mit erhöhter Krafteinwirkung auftreten. Die Inzidenz atypischer Femurfrakturen ist grundsätzlich gering, das relative Risiko scheint jedoch unter Langzeittherapie mit Bisphosphonaten erhöht, wenngleich eine Erhöhung des absoluten Frakturrisikos bisher nicht gezeigt werden konnte. Weitere Studien zu diesem Thema sind sicherlich erforderlich. Die bisher vorliegenden Daten sprechen jedoch für eine strenge Indikationsstellung sowie laufende Reevaluierung der Therapiestrategie aufgrund der mittlerweile größeren Auswahl an Optionen zur Behandlung der Osteoporose.
Summary
Atypical fractures under long term treatment with bisphosphonates (BP) have been reported, although no causal connection has been known. As an explanation the suppression of bone turnover with poor bone metabolism and consequently deterioration of bone microarchitecture has been suggested. In our case we describe retrospectively the course of radiological and clinical changes in a 75 years old female patient who has been treated with oral BP due to postmenopausal osteoporosis over 15 years. After 10 years of treatment she developed a cortical stress fracture in the subtrochanteric region of the femoral shaft, which finally caused a spontaneous atypical subtrochanteric fracture 5 years later.
