Zusammenfassung

Bei bis zu 8% aller Tumorerkrankungen manifestiert sich ein paraneoplastisches Syndrom. Die Pathogenese ist bislang im Detail bei den meisten paraneoplastischen Syndromen nicht geklärt; vom Tumor sezernierte Hormone, Peptide, Wachstumsfaktoren und Zytokine bzw. Kreuzreaktivitäten zwischen malignen und normalen Zellen werden als pathogenetisch relevante Faktoren diskutiert. Paraneoplastische Syndrome können neben endokrinen, hämatologischen, neurologischen, auch rheumatische Beschwerden zur Folge haben. Andererseits können rheumatische Symptome bei hämatologisch-onkologischen Erkrankungen, insbesondere lymphoproliferativen Systemerkrankungen auf eine Zellinvasion, Kompression und/oder direkte hämatologische Wirkungen zurückgeführt werden. Ziel dieses Beitrags ist es, einen Überblick über die vielfältigen Assoziationen von rheumatischen Symptomen und hämatologischen/onkologischen Erkrankungen zu geben, die in der klinischen Praxis von Bedeutung sein könnten.

Abstract

Paraneoplastic syndromes are observed in up to 8% of tumor patients. These disorders arise from tumor secretion of hormones, peptides, growth factors or cytokines or from immune cross-reactivity between malignant and normal tissues. Among many others paraneoplastic syndromes may also affect the rheumatologic system resulting in various musculoskeletal symptoms and/or syndromes. On the other hand, mainly hematological or lymphoproliferative diseases may also cause rheumatic symptoms by cell invasion or when affecting cellular elements of blood or the coagulation system. The aim of this article is to provide an overview of the various associations between rheumatic symptoms and hemato-oncological diseases which might be of importance in clinical practice.
