Zusammenfassung
Der akute Schmerz bei segmentaler Dysfunktion ist die ideale Indikation für manuelle Therapie, insbesondere für eine Manipulation, sofern keine Kontraindikationen vorliegen. Dieser körperprotektive Schmerz wird damit sehr rasch verschwinden. Chronischer Schmerz ist aber nicht einfach die zeitliche Fortsetzung des akuten Schmerzes, sondern stellt eine Änderung des Verhaltens der Nervenzellen ab dem 2. Neuron dar (sog. Neuroplastizität). Es gibt kein Medikament gegen den chronischen Schmerz. Manuelle Techniken können dagegen erfolgreich sein, wenn sie (innerhalb eines multimodalen Ansatzes) das Bindegewebe und die Muskulatur mit den damit verbundenen Faszienstrukturen berücksichtigt, die einen ganz anderen manuellen Ansatz erfordern als die Intervertebralgelenke, sehr langwierige und nicht immer schmerzfreie Release-Techniken.
Abstract
Acute pain in segmental dysfunction is an ideal indication for manual therapy, especially for impulse manipulation, as long as there are no contraindications. This body-protective pain will quickly disappear after treatment; however, chronic pain is not only the continuation of acute pain over a longer period but represents a substantial change in the behavior of neurons, starting with the second neuron (so-called neuroplasticity). There is no medication against chronic pain; nevertheless, manual techniques can be successful in a multimodal approach, if they take the connective and muscle tissues as well as the associated fascial structures into consideration. These tissues require a completely different manual approach in comparison to intervertebral joints, which is very time consuming and not always involving pain-free release techniques.
