Zusammenfassung

Verletzungen der posterolateralen Strukturen treten häufig in Kombination mit Verletzungen des hinteren Kreuzbands (HKB), seltener bei Rupturen des vorderen Kreuzbands (VKB) und sehr selten isoliert auf. Verschiedene Rekonstruktionstechniken sind beschrieben. Aufgrund der guten klinischen Ergebnisse sowie der verhältnismäßig einfachen und reproduzierbaren Technik hat sich die posterolaterale Stabilisierung nach Larson als häufig angewendetes Verfahren bei solchen Verletzungen etabliert. Dabei werden das laterale Kollateralband und das popliteofibulare Ligament mit einem freien Sehnentransplantat annähernd anatomisch rekonstruiert. In der Regel kommt dieses Verfahren in Kombination mit der Rekonstruktion eines oder beider Kreuzbänder zum Einsatz. Der folgende Beitrag schildert detailliert die von den Autoren modifizierte und angewendete Technik der posterolateralen Rekonstruktion.

Abstract

Injuries of the posterolateral structures are common in combination with injuries of the posterior cruciate ligament, less frequent with injuries of the anterior cruciate ligament and quite rare as isolated injuries. Different reconstruction techniques have been published. With regard to its good clinical results and its relatively straightforward and reproducible technique the Larson reconstruction has become a commonly performed procedure for these injuries. The lateral collateral ligament and the popliteofibular ligament are reconstructed approximately anatomically with an autologous tendon graft. Usually, the procedure is performed in combination with reconstruction of one or both cruciate ligaments. The following article describes the technique of posterolateral reconstruction as modified and performed by the authors in detail.
