Zusammenfassung

Hintergrund

Die Arthroskopie des Ellenbogens wird in der akuten Traumasituation sicherlich selten durchgeführt, und es bedarf einer gewissen Erfahrung des Operateurs im Umgang mit dieser Technik und dem Gelenk. Am ehesten wird die Arthroskopie in solchen Fällen zur Bestandsaufnahme der eingetretenen Gelenkpathologie mit Lavage des Hämarthros und Erfassung der Begleitverletzungen, wie knorpelige Abscherverletzungen oder freie Gelenkkörper (z. B. ein Koronoidspitzenfragment), sowie zur Beurteilung der ligamentären Instabilität angewandt. Der Effekt der Arthroskopie ist daher meist diagnostischer und minimal-invasiv resezierender Natur. Dieser Artikel soll aber auch auf die rekonstruktiven Möglichkeiten der Arthroskopie in der Traumasituation des Ellenbogens hinweisen. Hierzu gehört die arthroskopische Versorgung von Radiuskopffrakturen, Koronoidfrakturen, knöchernen Ausrissverletzungen des Ligamentum anulare am anterolateralen Koronoid, geeigneten Capitulumfrakturen, geeigneten distalen Humerus- und proximalen Olekranonfrakturen.

Fragestellung

Wie ist der aktuelle Stand der arthroskopisch gestützten Frakturversorgung am Ellenbogen? Handelt es sich hierbei bereits um ein Standardverfahren, oder ist es vergleichbar mit den offenen Techniken?

Methoden

Basierend auf einer selektiven Literaturrecherche mit dem Schwerpunkt auf Arbeiten zur arthroskopischen Frakturversorgung des Ellenbogens werden die Daten aus bisherigen Veröffentlichungen zu diesem Themenkomplex dargestellt und diskutiert. Ferner werden eigene Erfahrungen und Operationstechniken mit einbezogen.

Schlussfolgerung

Die arthroskopische bzw. arthroskopisch gestützte Frakturversorgung in der akuten Traumasituation kann unserer Ansicht nach durch die minimal-invasive Technik mit geringer Weichteiltraumatisierung einen großen Nutzen für die Patienten darstellen. Sie ist aber sicherlich bei der heutigen dünnen Datenlage noch nicht generell und flächendeckend zu empfehlen.

Abstract

Background

Arthroscopically assisted techniques in treatment of elbow fractures are rarely carried out and require a certain level of expertise of the surgeon regarding the technique of elbow arthroscopy and the elbow joint. In the acute trauma situation elbow arthroscopy is mostly used to remove loose bodies, to irrigate intra-articular hemorrhage and to evaluate ligamentous instability and is therefore used as a diagnostic tool or minimally invasive resection technique. This article describes the therapeutic reconstructive capabilities of elbow arthroscopy in the acute trauma situation, including arthroscopically assisted fracture treatment of radius head fractures, coronoid fractures, avulsion fractures of the anterolateral coronoid, some capitellum fractures, some fractures of the distal humerus and the proximal olecranon.

Objectives

What is the current state of the art in arthroscopically assisted techniques for the treatment of elbow fractures? Are these standard procedures or are they comparable to open procedures?

Methods

Based on a literature review regarding studies of arthroscopically assisted techniques for treatment of elbow fractures, data from the available literature are presented and discussed as well as the results of own experiences and techniques.

Conclusions

Arthroscopically assisted techniques for treatment of elbow fractures decrease soft tissue damage as a direct benefit for the patient. Due to limited data available no general recommendations for this procedure can be made.
