Zusammenfassung
Hintergrund
Wir berichten über einen 38-jährigen, aktiven Patienten, welcher aufgrund eines Ewing-Sarkoms im Jugendalter mit einer Tumorendoprothese versorgt wurde (Ersatz des distalen Femurs und der proximalen Tibia). Im weiteren Verlauf ist es zu mehreren (Teil-)Wechseloperationen gekommen. Vorstellig wurde er in unserer Klinik mit persistierenden Schmerzen des rechten Unterschenkels. In der durchgeführten Bildgebung bestätigte sich eine aseptische Lockerung der tibialen Komponente. Aufgrund seines langen Leidensweges lehnte der Patient einen weiteren Eingriff zur Erhaltung der Extremität ab. Vielmehr bestand der Wunsch nach einer Amputation.
Ziel der Arbeit
Mit einer klassischen, transfemoralen Amputation oberhalb der Femurkomponente wären wir das Risiko eines zu kurzen Stumpfes eingegangen, welches Schwierigkeiten angesichts der orthopädietechnischen Versorgung mit funktionellen Einschränkungen hätte bedeuten können.
Methode
Zur besseren Versorgung haben wir die Femurkomponente belassen, im Kniegelenk entkoppelt und somit eine biomechanisch überlegene Knieexartikulation simuliert.
Ergebnis
Es zeigte sich postoperativ mit der interimistischen Prothesenversorgung in den Zeit-Distanz-Parametern ein weitgehend harmonisches Gangbild, welches mit der finalen Versorgung noch verbessert werden kann. Die sagittalen Gelenkwinkel sind mit denen von herkömmlich Kniexartikulierten vergleichbar.
Diskussion
Bis dato ist kein vergleichbarer Fall einer solchen Alloarthroplastik beschrieben, welcher mit einer orthopädietechnischen Versorgung seine Mobilität wiedererlangen konnte. Im Ergebnis steht die Exartikulation im Bereich des endoprothetischen Kniegelenks unter Beibehaltung der femoralen Komponente der „gewöhnlichen“ Knieexartikulation in nichts nach.
Abstract
Background
We report an unusual case of a 38-year-old physically active patient, who suffered a Ewing sarcoma during adolescence and was therefore treated with a tumor endoprostheses ex domo (replacement of distal femur and proximal tibia). Since then he had undergone a couple of surgical interventions for revision and is currently suffering from persistent pain in the leg concerned. An aseptic loosening of the tibial component was evident after radiologic examinations. Because he had suffered for so long our patient refused further surgery to retain the affected limb and he preferred amputation.
Objectives
We would have taken the risk of a femoral residual limb that is much too short compared with a classical above-knee amputation. In this regard we saw difficulties in exoprosthetic treatment with functional limitations that could easily have influenced the outcome negatively.
Method
Consequently, we decoupled the tibial component from the femoral one within the knee joint and thus performed a knee disarticulation leaving the femoral component.
Results
Postoperatively, we found a mostly harmonious gait pattern with a pure mechanical interim prosthesis regarding time-distance parameters, which may be even further improved with the final prosthesis. Sagittal joint angles are comparable to conventionally knee exarticulated ones.
Discussion
To date, no comparable case of such an alloarthroplasty, which could regain mobility with an exoprosthetic treatment has been highlighted in the literature. In the result, a knee disarticulation within the area of a knee arthroplasty leaving the femoral component is not inferior to“ordinary” knee disarticulation.
