Zusammenfassung
Hintergrund
Faktoren, welche die Effektivität therapeutischer Maßnahmen zutreffend vorhersagen, werden als prädiktiv definiert. Welche Rolle diese in der metastasierten Situation gynäkologischer Malignome spielen, ist Gegenstand dieses Beitrags.
Ziel
Mit diesem evidenzbasierten Review soll die Rolle prädiktiver Faktoren bei gynäkologischen Tumoren beschrieben werden.
Material und Methoden
Mittels systematischer Literaturrecherche in Pubmed und manueller Recherche wurden relevante Publikationen zwischen 2005 und 2015 untersucht, laufende Studien wurden unter clinicaltrials.gov abgefragt. Suchbegriffe waren „vulvar cancer“, „cervical cancer“, „endometrial cancer“ und „ovarian cancer“ in Kombination mit „predictive“ und „therapy“.
Ergebnisse
Eine große Anzahl potenzieller Biomarker sind in den letzten Jahrzehnten in gynäkologischen Malignomen untersucht worden. Dabei sind lediglich das Epithelial Cell Adhesion Molecule (EpCAM) und mutiertes Brustkrebsgen 1 und 2 (BRCA1 und 2) die einzigen etablierten prädiktiven Faktoren, die einen nachgewiesenen Nutzen durch eine zielgerichtete Therapie vorhersagen können. Der prädiktive Wert des Östrogenrezeptors (ER) und des Progesteronrezpetors (PR) beim rezidivierten Endometriumkarzinom wird bis heute kontrovers diskutiert. Derzeit werden weitere Biomarker in laufenden Studien auf ihren prädiktiven Wert hin untersucht.
Schlussfolgerung
Prädiktive Faktoren werden beim rezidivierten Ovarialkarzinom und mit gewissen Einschränkungen beim rezidivierten Endometriumkarzinom verwendet, um zielgerichtete Therapien erfolgreich einzusetzen. Eine große Anzahl an Biomarkern wird derzeit untersucht.
Abstract
Background
Predictive biomarkers are associated with response to a given therapy.
Objective
The aim of this evidence-based review was to define the predictive relevance of biomarkers in gynecological tumors.
Materials and methods
A systematic search in PubMed and a manual search were carried out to review relevant articles published between 2005 and 2015. Ongoing studies were identified with clinicaltrials.gov. Key words used were “vulvar cancer”, “cervical cancer”, “endometrial cancer”, “ovarian cancer” in combination with “predictive” and “therapy”.
Results
A huge number of biomarkers have been evaluated in gynecological malignancies in the last few decades. However, epithelial cell adhesion protein (EpCAM) and mutated breast cancer 1 and 2 (BRCA1 and 2) represent the only two biomarkers to predict a proven benefit for a given targeted therapy in patients with recurrent ovarian cancer. To date the predictive impact of estrogen receptor (ER) and progesterone receptor (PR) remains controversial in endometrial cancer. Currently, further biomarkers are under investigation.
Conclusion
Predictive factors are especially used in recurrent ovarian cancer and with some limitation in recurrent endometrial cancer to select patients for targeted therapies. A large number of biomarkers are currently under investigation.
