Hintergrund
Infektiologische und sonographische Vorsorgeuntersuchungen sowie die präkonzeptionelle Beratung haben die Geburtsmedizin wesentlich beeinflusst.
Fragestellung
Definiert werden sollen die aktuell gültigen Inhalte der infektiologischen und sonographischen Vorsorgekonzepte, abgeglichen werden soll ggf. mit schon bestehenden evidenzbasierten Handlungskonzepten.
Material und Methoden
Autorenzentrierte subjektive Auflistung, Bewertung und Diskussion der derzeitigen staatlichen Vorsorgekonzepte im Kontext der hierzu bisher verfügbaren Grundlagenarbeiten und Expertenempfehlungen.
Ergebnisse
Trotz ähnlich strukturierter und dokumentierter Infektions- und Ultraschallvorsorgeprogramme einschließlich einer präkonzeptionellen Impf- und Ernährungsberatung sind die messbaren Auswirkungen in den nationalen Perinatalerhebungen sehr variabel. In den vergangenen 10 Jahren kommen zunehmend komplexere und effektivere Methoden im nichtinvasiven vorgeburtlichen Screening auf Chromosomenstörungen zur Anwendung. Durch die seit etwa 2 Jahren kommerziell verfügbare nichtinvasive pränatale Testung (NIPT) von zellfreier fetaler DNA aus maternalem Blut kommt es derzeit zu einem vollständigen Paradigmenwechsel.
Schlussfolgerungen
Bereits zu Beginn der Schwangerschaft sollte die werdende Mutter auf die gesetzlich vorgesehenen Vorsorgeoptionen hingewiesen werden, um nach ärztlicher Beratung entscheiden zu können, ob sie eine je nach Land eventuell sogar zusatzkostenpflichtigen Ergänzungsuntersuchung wünscht.
Abstract
Background
The implementation of infectious and sonographic screening during pregnancy has had an important impact on perinatal medicine.
Objectives
Current criteria as well as the possible benefits of infectious and sonographic screening concepts during pregnancy are defined, discussed, and compared with the evidence of afflicting or supporting government-backed screening recommendations.
Materials and methods
Quality and strength of recent literature reviews, national screening guidelines, and published expert opinions including their applicability are considered and subjectively addressed by the authors.
Results
Despite the introduction of screening and prevention concepts for both maternal infections and fetal malformations including measures such as preconceptional supplementation of folic acid or maternal vaccination, the effectiveness of these core interventions to improve pregnancy outcome remains partly unclear. In the last decade, noninvasive prenatal screening modalities for aneuploidies have become increasingly complex and effective. In the last 2 years, the rapid progress in harnessing cell-free fetal DNA from maternal blood, which is also called noninvasive prenatal testing (NIPT), has led to a change of paradigm in the current approach of antenatal genetic screening formerly based on maternal age combined with other first trimester markers.
Conclusion
Even at the early stages of pregnancy, pregnant women should be counselled about the ease and effectiveness of government-based antenatal screening recommendations for both maternal infections and fetal malformations. This provides them the opportunity to opt for further investigations. However, some costs for additional antenatal screening are not (yet) covered by all national health insurance systems.
