Zusammenfassung

Ältere Patientinnen mit Brustkrebs werden seltener leitliniengerecht therapiert und seltener in klinische Studien eingeschlossen. Dies führt zu einer Reduktion der rezidivfreien Zeit und des Gesamtüberlebens. Es gilt, die Patientinnen zu definieren, die sich für eine leitliniengerechte Therapie eignen und den nicht geeigneten Patientinnen die bestmögliche „reduzierte“ Therapie anzubieten. Dazu müssen vernachlässigte Aspekte wie körperliche Fitness und Behinderung im Alltag, soziale Netzwerke, Lebensqualität und allgemeine Komorbiditäten verstärkt berücksichtigt werden. Hierzu kann ein angepasstes geriatrisches Assessment dienen. In diesem Artikel wird eine Übersicht über die aktuelle Datenlage bezüglich Therapiemodalitäten und -nebenwirkungen gegeben sowie eine Auswahl geriatrischer Assessments aufgezeigt.

Abstract

Standard treatment of breast cancer patients decreases with age, and older women are usually excluded from clinical trials. This results in decreased overall survival and disease-free survival. To improve treatment options for the older person, it is important to identify those who would benefit from an aggressive standard treatment and those who would not. In older women with breast cancer, physical function and disability, social support, quality of life, and overall comorbidity are important clinical parameters that, to a great extent, determine treatment success. This article gives an overview of specific treatment modalities, typical adverse events, and methods of geriatric assessment.
