Wann ist die operative Versorgung von Metastasen beim Nierenzellkarzinom angezeigt und gibt es eine wissenschaftliche Grundlage hierfür?

Is surgical treatment ever indicated in metastatic renal cell carcinoma and if so, based on which scientific rationale?

Zusammenfassung

Hintergrund

Metastasen des Nierenzellkarzinoms sind häufig. Die operative Versorgung von syn- wie metachronen Metastasen eines Nierenzellkarzinoms kann aus zweierlei Aspekten heraus sinnvoll sein: einerseits aus Gründen der Palliation, wofür v. a. die eingeschränkte Strahlensensibilität des Nierenzellkarzinoms spricht, andererseits im Hinblick auf eine Verlängerung der Überlebenszeit, wofür v. a. die teils relativ günstige Prognose einiger Metastasierungsmuster spricht.

Ziel der Arbeit

Diese Übersicht soll die wesentliche Evidenz zu beiden Aspekten beleuchten und diskutieren. Es erfolgte eine Literaturrecherche in PubMed sowie die Sichtung der entsprechenden Leitlinien.

Ergebnisse und Diskussion

Eine palliative Resektion ist in erster Linie bei Kompressionssymptomen durch Metastasen der Wirbelsäule sinnvoll. Hier sollte dann eine adjuvante Strahlentherapie erfolgen. Die Resektion ist aber v. a. aus Gründen der Verlängerung des tumorspezifischen Überlebens angezeigt. Dies gilt wohl eigentlich für alle resektablen Metastasierungsmuster – solitäre, multiple, syn- und metachrone Filiae aller Organsysteme. Die Voraussetzung für einen positiven Effekt auf das Überleben ist allerdings eine komplette Resektion. Eine dezidiert günstige Prognose besteht bei solitären Lungenmetastasen ohne begleitende hiläre oder mediastinale Lymphknotenmetastasen mit einem späten metachronen Auftreten nach Tumornephrektomie. Die komplette Resektion kann hier u. U. als kurativ angesehen werden. Eine neo- oder adjuvante medikamentöse Therapie oder Bestrahlung ist bislang nicht etabliert. Erscheint eine komplette Resektabilität nicht gegeben, kann eine stereotaktische Radiatio als Alternative erwogen werden.

Abstract

Background

Metastasis is a common event in renal cell carcinoma. Surgical resection of metastases may be feasible in two scenarios: aiming at palliation, which may be feasible due to reduced radiosensitivity of renal cell cancer, and aiming at prolonging survival, which may be feasible given the rather good prognosis of some patterns of metastasis.

Objective

This review intends to reflect on current evidence for surgical resection of metastases in both scenarios. The literature was searched in PubMed and respective guidelines were reviewed.

Results and conclusions

Palliative resection is mainly feasible to control symptoms like spinal compression; adjuvant radiation is advisable. Resection is markedly feasible, however, for improvement of cancer-specific survival in probably all resectable patterns of metastasis—solitary, multiple, syn- and metachronous, and in all organs—provided complete resection can be achieved. A fairly good prognosis is seen in solitary pulmonary metastasis without concomitant hilar or mediastinal lymph node metastasis and a metachronous appearance following long recurrence-free survival after tumor nephrectomy; complete resection may be considered curative in certain cases. Neo- or adjuvant medical therapy or radiation is not established. In cases lacking complete resectability, stereotactic radiation may be considered as an alternative.
