Zusammenfassung
Akute psychomotorische Erregungszustände bedürfen als psychiatrische Notfälle einer unmittelbaren Intervention und Behandlung, da sie z. T. lebensbedrohlich verlaufen können. Sie sind in psychiatrischen Kliniken, aber auch in Allgemeinkrankenhäusern, im Rettungsdienst oder in Praxen nicht selten und für alle Beteiligten belastend. Ziel dieser Arbeit ist es, die wichtigsten epidemiologischen und diagnostischen Aspekte von Erregungszuständen darzustellen, auch hinsichtlich ihrer differenzialdiagnostischen Bedeutung, und rasche, effektive Behandlungs- und Bewältigungsstrategien aufzuzeigen. Kontrollierte Untersuchungen und verlässliche Daten zur Häufigkeit und Versorgung von Erregungszuständen in psychiatrischen und anderen Institutionen sind rar. Aus den vorliegenden Studien lassen sich jedoch Hinweise ableiten, dass eine Optimierung bezüglich der Diagnostik und Therapie psychomotorischer Erregungs- und Unruhezustände erforderlich ist. Die Behandlung solcher psychiatrischer Notfallsituationen stellt hohe Anforderungen nicht nur an das psychiatrische Wissen, sondern auch an die Persönlichkeit und das Verhalten des Arztes. Zur therapeutischen Basis zählt die verlässliche und vertrauensbildende Beziehung zum Patienten, zum Handlungsrepertoire das ebenso geduldige wie beruhigende Zureden. Klare und schnelle therapeutische Entscheidungen unter Kenntnis effektiver medikamentöser Behandlungsstrategien führen in der Regel zum raschen Abklingen der akuten Erregungssymptomatik.
Summary
Acute agitation psychiatric emergencies as frequently occur in psychiatric as well as in non-psychiatric settings, such as general hospitals, specialized clinics, emergency services and private practices. Psychiatric emergencies can be life-threatening and necessitate immediate treatment. This article presents the core symptomatology, differential diagnoses and treatment options of acute agitation emergencies. Case control studies and reliable data regarding prevalence and treatment of acute agitation in psychiatric and general hospitals or private practices are sparse. Existing evidence suggests that optimization of diagnosis and therapy of psychiatric emergencies, such as acute agitation is warranted. Treatment of acute agitation, psychological distress and other psychiatric emergencies are highly demanding regarding psychiatric expertise and concerning the personality and behavior of the therapist. The basis of therapy comprises the ability to form a stable and trustworthy relationship with the patient as well as to patiently calm down agitated patients. Unambiguous and rapid decision-making that takes effective pharmacological treatment options into account usually leads to swift amelioration of the acute symptomatology.
