Zusammenfassung
Burnout ist ein arbeitspsychologisches Konzept, das die symptomatischen Auswirkungen langfristiger Anpassungsvorgänge an die Belastungen am Arbeitsplatz erfasst. Es ist keine Diagnose in den psychiatrischen Klassifikationssystemen der Internationalen Klassifikation der Krankheiten (ICD-10) oder des Diagnostischen und Statistischen Manuals Psychischer Störungen (DSM-IV-TR). In der ICD-10 wird Burnout aber als ein bedeutsamer Faktor anerkannt, der Probleme in der Lebensbewältigung abbildet, den Gesundheitszustand beeinflusst und zur Inanspruchnahme von Gesundheitsdiensten führen kann. Burnout kann eine vorübergehende oder aber eine anhaltende Arbeitsüberlastung sein, als Risikozustand für ernste psychische und somatische Krankheiten angesehen werden oder ein integrales Symptombild psychischer und somatischer Krankheiten darstellen. Das Kardinalsymptom der Erschöpfung oder anhaltenden Müdigkeit muss in einer psychiatrischen Differenzialdiagnostik v. a. hinsichtlich depressiver, Angst- und somatoformer Störungen bewertet werden. In den meisten Fällen eines ausgeprägten Burnouts liegt eine depressive Störung vor, die als solche diagnostisch erkannt und auch konsequent psychotherapeutisch und/oder pharmakologisch behandelt werden muss. Eine ätiopathogenetische Konzeptualisierung von Burnout sollte innerhalb eines multifaktoriellen biopsychosozialen Krankheitsmodells erfolgen. Schlussfolgerungen für die ärztliche Praxis werden formuliert.
Abstract
Burnout is primarily conceptualized by work psychology. It describes symptoms resulting from a long-standing, finally derailing adjustment to work-related stressors. Burnout is not a proper diagnosis according to traditional classification systems. However, ICD-10 considers burnout as a significant factor representing major personal problems that have impact on health status and illness behaviour. Burnout may be considered either as a transitional or persisting adjustment reaction to work-related stress, a condition of increased risk regarding to serious mental illnesses and physical diseases, or an integral syndrome of these various conditions. The core symptom of exhaustion or persisting tiredness must be carefully assessed in respect of depressive, anxiety and somatoform disorders from the perspective of psychiatric differential diagnosis. In most cases of a serious burnout the diagnosis of major depression can be established and should lead to proper psychotherapeutic and/or pharmacological treatments. Any aetiopathogenetic evaluation may be favourably done within a multifactorial biopsychosocial model. Consequences for medical care will be described.
