Zusammenfassung
Eine 42-jährige Patientin stellte sich mit dem Verdacht auf Leberzirrhose und begleitendem Aszites bei bekannter Polyglobulie vor. Laborchemisch fand sich eine reduzierte Syntheseleistung der Leber sowie eine deutliche Erhöhung aller 3 Reihen der Hämatopoese. Histologisch wurden eine Leberfibrose sowie in der Knochenmarksbiopsie eine Polycythaemia vera gesichert. Die häufigsten Ursachen einer Leberfibrose wurden serologisch ausgeschlossen. Dopplersonographisch zeigte sich ein Verschluss der rechten Lebervene im Sinne eines Budd-Chiari-Syndroms. Chronisch myeloproliferative Erkrankungen sind eine häufige Ursache des Budd-Chiari-Syndroms. Neben der fulminanten ist auch eine protrahierte Verlaufsform möglich, die zu einer progredienten Schädigung der Leber führen kann. Unter Aderlasstherapie sowie Medikation mit niedrig dosierter Azetylsalizylsäure und Anagrelide kam es bei unserer Patientin zu einem anhaltenden Abfall der erhöhten Blutparameter und keiner erneuten hepatischen Dekompensation.
Abstract
A 42-year-old patient was admitted to hospital because of ascites and polyglobulia. Laboratory tests revealed reduced liver function and a significant elevation of all three hematopoietic cell lines. Liver fibrosis and polycythemia vera were diagnosed by histologic examination. The most frequent causes for liver fibrosis were serologically excluded. Ultrasound combined with Doppler imaging revealed an obstruction of the right hepatic vein, which was indicative of Budd-Chiari syndrome. BCS can occur under fulminant and nonfulminant conditions, which can result in progressive damage of the liver. Phlebotomy and combined therapy with low-dose aspirin and anagrelide achieved permanent reduction of the elevated blood parameters. In the follow-up the patient’s clinical course was stable without hepatic decompensation.
