Zusammenfassung

Hintergrund

Heiserkeit, die auf einem unzureichenden Stimmlippenschluss beruht, kann durch Injektion von Füllsubstanzen in die betroffene Stimmlippe behandelt werden. Durch eine Verbesserung der zur Verfügung stehenden Injektionsmaterialien und Erschließung weiterer operativer Zugangswege hat sich das Spektrum der Behandlungsmöglichkeiten erweitert. Die Behandlung kann in Oberflächenanästhesie oder Narkose durchgeführt werden. Eine sorgfältige Indikationsstellung ist für den Behandlungserfolg ausschlaggebend.

Ziel

Es werden derzeit verwendete Injektionsmaterialien, operative Zugangswege und differenzialtherapeutische Möglichkeiten beschrieben.

Material und Methoden

Für die Übersicht wurde eine Literaturrecherche durchgeführt und Erfahrungen aus über 500 eigenen Stimmlippeninjektionen zusammengefasst.

Ergebnisse und Diskussion

Durch gezielten Einsatz der zur Verfügung stehenden Injektionsmaterialien kann eine befriedigende Stimmverbesserung bei geringer Patientenbelastung erreicht werden. Voraussetzung für den Behandlungserfolg ist die geeignete Indikationsstellung. Wünschenswert sind eine weitere Verbesserung der Injektionsmaterialien hinsichtlich gut einschätzbarer Verweildauer im Gewebe und eine Verbesserung der Biokompatibilität.

Abstract

Background

Hoarseness due to insufficient glottal closure can be treated by injection laryngoplasty (augmentation). Vocal fold injections can be performed as an in-office procedure or under general anesthesia. As injection materials have recently improved and different injection approaches are available, injection laryngoplasties are performed much more frequently.

Objectives

Advantages and disadvantages of injection materials, laryngeal approaches, and differential indications are described.

Materials and methods

Literature research and experience of more than 500 laryngeal injection procedures are provided.

Results and conclusions

Sophisticated use of available injection materials can provide a satisfying improvement of voice function with minimal patient discomfort. Adequate indication is mandatory for a favorable outcome. Further improvement regarding biocompatibility and the duration of injection materials is desirable.
