Operationsziel
Wiederherstellung der aktiven Handgelenksstreckung, Daumenstreckung und Langfingerstreckung durch Muskeltransposition.
Indikationen
Irreparable periphere Radialisparese. Erkrankungen peripherer Nerven, Muskel-/Sehnenverletzung. Internal Splint bei hoher Radialisparese.
Kontraindikationen
Reversible distale Radialisparese. Fehlen geeigneter Donor-Muskeln, spastische Parese. Fehlende passive Gelenkbeweglichkeit. Narbige Weichteilverhältnisse. Spendermuskel mit eingeschränkter Funktion. Reinnervierte Muskelsehneneinheiten. Progressive Muskel- oder Nervenerkrankung. Mangelnde Compliance.
Operationstechnik
Transposition von M. flexor carpi ulnaris, M. palmaris longus und M. pronator teres. Sehnendurchflechtung von M. pronator teres mit M. extensor carpi radialis brevis, M. extensor digitorum communis mit M. flexor carpi ulnaris. Sehnendurchflechtung von M. extensor pollicis longus mit M. palmaris longus nach Mobilisation aus 3. Strecksehnenfach und Umlagerung. Durchflechtungen nach Pulvertaft.
Weiterbehandlung
Ruhigstellung für 3 Wochen in Entlastungshaltung in palmarer Unterarmschiene. Anschließend Schiene zur Nacht für weitere 2 Wochen. Intensive krankengymnastische Nachbehandlung für 4–6 Wochen mit Beginn ab der 3. postoperativen Woche.
Ergebnisse
Die beschriebene motorische Ersatzplastik kam in einem Zeitraum von 14 Jahren in 12 Fällen zur Anwendung. In 9 Fällen lag eine hohe Radialisparese vor. In Übereinstimmung mit der aktuellen Literatur kann die beschriebene motorische Ersatzplastik als zuverlässiges Verfahren angesehen werden.
Abstract
Objective
Restoration of active extension of wrist, thumb and digits by muscle-tendon transposition.
Indications
Radial nerve palsy due to peripheral nerve injury. Peripheral nerve disease. Muscle or tendon injury. Restoration of wrist extension in high radial nerve palsy.
Contraindications
Reversible distal radial nerve palsy, absence of suitable donor muscles, spasticity, limited range of motion of affected joints, extensive scarring and inappropriate soft tissue conditions, unjustifiable loss of function at donor site. Reinnervated donor muscles, progressive muscle disease, insufficient patient compliance.
Surgical technique
Dissection of the flexor carpi ulnaris, palmaris longus and pronator teres tendon insertion. Transposition of the tendons. Interweaving of tendons of the pronator teres and extensor carpi radialis brevis muscles, the extensor digitorum communis and flexor carpi ulnaris muscles, as well as the extensor pollicis longus and palmaris longus muscles using the Pulvertaft technique.
Postoperative management
3 Weeks immobilization in forearm splint. Additional immobilization for 2 weeks at night. Subsequently, intensive physical and occupational therapy for another 4–6 weeks is required, starting 3 weeks postoperatively.
Results
The procedure was carried out in 12 patients over the past 14 years. We treated proximal radial nerve palsy in nine cases. In accordance with the current medical literature, we consider the described motor replacement surgery a reliable procedure.
