Zusammenfassung
Das EKG ist in der Differenzialdiagnostik bei akutem Koronarsyndrom eine äußerst wichtige Untersuchung, die unmittelbar bei Kontaktaufnahme durchgeführt werden muss. Dabei wird bei ST-Hebungs-Infarkt (STEMI) eine sofortige Reperfusionstherapie empfohlen, da man von einem kompletten Verschluss mindestens einer Koronararterie ausgehen muss. Im EKG sollten sämtliche Zeichen, die hinweisend auf einen kompletten Koronarverschluss sind, erkannt werden. Die weniger bekannten EKG-Zeichen des Vorderwandinfarkts sollten von allen ärztlich Tätigen erkannt werden, die mit Patienten in der Akutphase in Kontakt treten können. Das in dieser Arbeit präsentierte Kompendium dieser EKG-Veränderungen kann als Entscheidungshilfe herangezogen werden, wenn das EKG von Personal befundet wird, das nicht regelmäßig EKGs auswertet.
Abstract
The ECG is a very important diagnostic tool if an acute coronary syndrome is suspected. It should be performed immediately when medical staff contacts the patient. If an ST elevation myocardial infarction (STEMI) is diagnosed, immediate reperfusion of the occluded vessel should be the primary goal. Therefore, even subtle signs of an anterior wall infarction diagnosed by the ECG which lead to suspicion of complete occlusion of a coronary artery should be known and diagnosed by all physicians who may have contact with these types of patients. The ECG variations presented in this article can be used as a sample for medical personnel who are not familiar with these changes and do not routinely interpret ECGs.
