Zusammenfassung
Die Adipositas stellt aufgrund ihrer epidemischen Verbreitung eine der zentralen Herausforderungen für unser Gesundheitssystem dar. Spätestens ab einem Body-Mass-Index (BMI) von 30 kg/m2 bzw. ab einem BMI von 25 kg/m2 und gleichzeitigem Vorliegen von Begleiterkrankungen ist eine gewichtssenkende Behandlung zu erwägen. Heute steht ein breites Spektrum von Therapiemaßnahmen zur Gewichtssenkung zur Verfügung, deren Wirksamkeit durch kontrollierte Studien belegt ist. Die Primärbehandlung zielt auf eine Lebensstiländerung mit energieärmerer Kost und mehr körperlicher Bewegung. Im Rahmen einer Stufentherapie kommen auch sehr niedrigkalorische Diäten, gewichtssenkende Medikamente und – bei extremer therapieresistenter Adipositas – chirurgische Maßnahmen in Betracht. Stets ist darauf zu achten, dass die Therapiemaßnahmen auf die Lebenswelt des Patienten zugeschnitten sind und dieser befähigt ist und ermutigt wird, die Eigenverantwortung für sein Gewichtsmanagement zu übernehmen. In Deutschland besteht erheblicher Handlungsbedarf, um die strukturellen Voraussetzungen für eine wirksame Adipositastherapie zu verbessern.
Abstract
Obesity is an epidemic and, thereby, represents one of the central challenges for the German health-care system. A weight-reducing treatment should be started at a body mass index (BMI) ≥ 30 kg/m2 or at a BMI ≥ 25 kg/m2 if clinically significant comorbidities are present. Currently, a broad range of evidence-based treatment options for weight reduction is available. The primary treatment aims at lifestyle modification by adopting an energy-reduced diet and a higher level of physical activity. In the escalation strategy, other options such as very-low-calorie diets, weight-reducing drugs and – in morbidly obese patients – bariatric surgery should be considered. All treatment measures should be tailored to the individual requirements, wishes and facilities of the patient. In addition, the patient should be encouraged to take responsibility for his/her weight management. However, there is urgent need to improve the structural preconditions for effective obesity management in Germany.
