Applications cliniques de la nalbuphine

Résumé

Les applications cliniques de la naibuphine, analgésique morphinique de type agoniste-antagoniste, sont revues au regard de la littérature. L'action particulière de la nalbuphine sur les récepteurs μ et κ, et ses propriétés pharmacocinétiques expliquent en partie ses effets cliniques et l'existance d'un effet plafond. Une sédation associée dans 10 à 40% des cas est son principal effet secondaire, alors que les répercussions cardiorespiratoires sont négligeables. La nalbuphine est utilisée dans toute la période périopératoire, permettant d'instaurer une véritable analgésie préventive, ainsi que pour les douleurs des urgences, de l'obstétrique, en réanimation, la douleur chronique associée au cancer, l'infarctus du myocarde. C'est un antalgique particulièrement intéressant chez l'enfant.

Summary

Clinical applictions of nalbuphine, a strong agonist/antagonist opioid, are rewieved with regard to the results of several studies. The specific activity of nalbuphine at μ and κ opioid receptors and its pharmacokinetics are suggesting some of its clinical properties and specially the ceillingeffect. Sedation was observed in 10 to 40% of patients as the main side effect. With regard to respiratory and hemodynamic changes, one found little degree of associated events. Clinical use of nalbuphine is emphasized in perioperative preemptive analgesia, emergency care, obstetrics, intensive therapy and chronic pain. Nalbuphine is a very suitable and reliable analgesic agent in pediatrics.
