Additional information

This is a revised version of a paper which I read at the University of colorado in July 1975; I gained a great deal from the lively discussion there. I would like to thank especially William Reinhardt, from the Department of Mathematics, who read the paper and made extensive comments. Many of the changes I made have been the direct result of his very useful remarks. The changes I did not make are, of course, no fault of his.
