Arterial surgery of the upper extremity

Abstract

Over a 10-year period, 116 arterial operations in the upper extremity were performed. Sixteen patients had suffered from trauma, 52 from gross embolism, and 48 from chronic ischemia. The groups differ in age, duration of symptoms, and localization of the lesion. In patients with chronic ischemia, the most common lesion was a left-sided subclavian stenosis, with microembolization to the fingers as the most prevalent symptom. Several types of reconstructions were used with satisfactory results. One patient in each group underwent amputation. The mortality rate in the group of patients with gross embolization was high, both postoperatively and during follow-up.

Résumé

Au cours d'une période de 10 ans 116 opérations artérielles au niveau du membre supérieur ont été pratiquées: 16 pour lésions traumatiques, 52 pour embolies artérielles et 48 pour ischémie chronique. Elles ont donné lieu à cette étude, ces interventions ayant été pratiquées chez des sujets d'âge variable, pour des troubles de durée inégale et pour des lésions de localisation différente.

Chez les malades atteints d'ischémie chronique la lésion la plus fréquente répondait à une sténose de la sous-clavière gauche associée à des micro-embolies des doigts. Différents types d'interventions furent pratiqués. Dans chacun des trois groupes un malade a dû être amputé. La mortalité fut importante chez les opérés qui présentaient une embolie artérielle massive soit immédiatement après l'intervention soit encore secondairement à celle-ci.

Resumen

Aunque relativamente raras, las enfermedades isquémicas de las extremidades superiores han merecido un interés creciente en el curso de los últimos años; tales enfermedades o lesiones difieren, en muchos aspectos, de las manifestaciones isquémicas de las extremidades inferiores.

Se analizaron 116 operaciones arteriales sobre las extremidades superiores efectuadas durante un período de 10 años. Dieciseis pacientes padecieron trauma, 52 embolia mayor y 48 isquemia crónica. Los grupos difieren en cuanto a edad, duración de los síntomas y localización de la lesión. La lesión más frecuente en los pacientes con isquemia crónica fue una estenosis subclavia izquierda con microembolización a los dedos como manifestación predominante. Diferentes tipos de reconstrucción fueron utilizados. Un paciente en cada grupo tuvo que ser sometido a amputación. La mortalidad en los pacientes con embolización mayor fue elevada, tanto en el período postoperatorio (30 días), como en el de seguimiento a largo plazo (2 a 96 meses).
