Esophageal dysfunction and its pathogenesis in progressive systemic sclerosis

Zusammenfassung

Bei 25 Patienten mit progressiver Sklerodermie wurde prospektiv die Frage der Oesophagusbeteiligung anhand subjektiver Symptome und radiologischer sowie manometrischer Kriterien untersucht. Bei allen Patienten waren röntgenologisch und/oder manometrisch Funktionsstörungen nachweisbar: röntgenologisch in 18 (72%), manometrisch in 22 (88%) Fällen. Dysphagie bestand nur bei 11 Patienten (44%). 6 Patienten (24%) waren trotz ausgeprägter objektiver Oesophagusveränderungen subjektiv völlig beschwerdefrei. Das Fehlen oesophagealer Symptome schließt damit einen fortgeschrittenen Befall der Speiseröhre nicht aus. Zur Frage der Pathogenese dieser Funktionsstörung wurde bei 12 dieser Patienten und bei ebensovielen Kontrollpersonen randomisiert die Wirkung von 3,5 µg/kg Carbachol und von 0,6 µg/kg Pentagastrin i.m. auf den Ruhedruck des unteren Oesophagussphinkters, auf die Amplitude und auf die Dauer der peristaltischen Kontraktion des distalen Oesophagus untersucht. Aufgrund der Ergebnisse kann eine primär myogene Genese der Funktionsstörung durch Atrophie und Sklerose der glatten Oesophagusmuskulatur angenommen werden.

Summary

In 25 patients with progressive systemic sclerosis esophageal involvement was studied prospectively by analysing subjective symptoms and radiological and manometric criteria. In all patients abnormal motility could be demonstrated by X-ray and/or manometry: radiologically in 18 (72%), manometrically in 22 (88%) cases. Dysphagia was present only in 11 patients (44%). Six patients (24%) had no subjective symptoms in spite of severe objective esophageal abnormalities. Therefore, the absence of esophageal symptoms does not exclude advanced affliction of the esophagus. To determine the pathogenesis of esophageal dysfunction the effects of 3.5 µg/kg carbachol and of 0.6 µg/kg pentagastrin i.m. on the lower esophageal sphincter pressure (LESP), on the amplitude and on the duration of peristaltic esophageal contractions were studied at random in 12 of the patients and 12 normal controls. The results suggest a primary myogenic genesis of the motor abnormalities by atrophy and sclerosis of esophageal smooth muscle.
