Abstract
Reviewing 24197 autopsies performed between 1954 and 1971 and 1227 kidneys removed at operation during the last 10 years we found 54 cases of multicystic kidney (MK). This condition is rare in adults (1:2400) and more frequent in young children (1:93). Malformations of the other kidney or of the lower urinary tract were present in 57% of the autopsy material and in 29% of the kidneys removed at surgery. Other malformations could only be found in the autopsy material (57%).
Prognosis is good only in cases of unilateral MK with an atretic or normal ureter and if the children affected show normal BUN values and pyuria is absent. MK was unilateral in all patients reaching adult life, and with the exception of 2 cases with atresia, the ureter was normal.
Marked pyelonephritis is rare in MK (11%) and occurs more frequently in partially dysplastic kidneys. If malformations of the lower urinary tract are also present pyelonephritis is often more pronounced in the kidney showing no congenital malformation. A partially dysplastic kidney with chronic inflammation can cause renal hypertension.
The MK gives no contrast on Roentgen plates, while the partially dysplastic kidney may give a slight shadow. MK should be looked for in children with malformations of ears, limbs and heart.
The lack of induction of the metanephrogenic Anlage by the ureteral bud is considered the most important pathogenic factor in MK and all teratogenetic substances can be considered as primary causative agents.
Zusammenfassung
Bei der Durchsicht des Sektionsgutes von 1954–1971 (24197 Fälle) und der in den letzten 10 Jahren eingesandten Nierenoperations-präparate (1227 Nieren) wurden 54 Nierendysplasien (ND) gefunden. Die ND ist selten bei Adulten (1:2400), häufig bei Säuglingen (1:93). Kombination der ND mit Mißbildungen der Gegenniere und/oder des unteren Harntraktes bestand in 57% der Sektions- und 29% der Nephrektomiefälle. Zusätzliche Körpermißbildungen wurden nur im Sektionsgut beobachtet (57%).
Die Prognose ist nur bei einseitiger ND mit atretischem oder normalem Ureter gut. Kinder mit diesem Befund wiesen normalen Harnstoffwert auf, und Pyurie fehlte. Fälle im Erwachsenenalter zeigten immer einseitige ND mit meist normalem oder seltener atretischem Ureter.
Schwere Pyelonephritis in dysplastischen Nieren ist selten (11%) und kommt überwiegend in partiell dysplastischen Nieren vor. Bestehen gleichzeitig Mißbildungen des unteren Harntraktes, wird besonders auch die Gegenniere schwer entzündlich verändert. Eine entzündlich befallene partielle Dysplasie kann eine renale Hypertonie erzeugen.
Radiologisch ist die totale ND stumm, bei partieller kann eine schwache Anfärbung erfolgen. Im Kindesalter ist bei Ohr-, Extremitäten- und Herzmißbildungen nach ND zu fahnden.
Wichtigster pathogenetischer Faktor für das Entstehen einer ND ist mangelhafte Induktion des metanephrogenen Blastems durch den Uretersproß. Als ursächliches Agens kommen alle terratogen wirksamen Noxen in Frage.
