Abstract
The coagulability of ACD blood, which we use for exchange transfusion, is very labile. Platelet counts and fibrinogen are considerably reduced. The danger of extensive coagulapathy or pancoagulapathy must be appreciated, since blood with a potential disturbance of coagulation is being used to replace blood of the newborn already involved with physiological coagulapathy.
Exchange transfusion was performed with 550–600 ml of ACD blood in each of 8 newborns after normal delivery.
Immediately after the end of exchange transfusion 2 mg of vitamin K1 in 20 ml of 5% glucose was given by slow i.v. injection in 3 cases and 2 mg of vitamin K2 was given instead of vitamin K1 in 5 cases.
Blood was obtained before administration and 12 hrs after administration for the TEG (Thromboelastogram) test. The alteration of the reaction time was defined as:
In the group treated with V K2, the change in r, tended to be more pronounced than that in the group treated with V K1.
It was thus suggested that the action of V K2 was more rapid than that of V K1.
Zusammenfassung
Die Gerinnungsfähigkeit von ACD-Blut, mit dem wir Austauschtransfusionen ausführen, ist sehr schwach.
Thrombocyten und Fibrinogen wiesen einen beträchtlichen Abfall auf, was auf eine Störung in der Gerinnungsfähigkeit schließen ließ.
Die Gefahr einer Pancoagulopathie kann jedoch vorausgeseben werden, weil ACD-Blut mit einer gestörten Gerinnungsfähigkeit statt des Blutes der Neugeborenen mit bereits bestehender physiologischer Verminderung der Blutgerinnugsfaktoren verwandt wird.
Bei 8 Neugeborenen wurde nach Normalgeburt die Blutaustauschübertragung mit 550–600 ml ACD-Blut durchgeführt.
Gleich nach Austauschtransfusion wurde in 3 Fällen 2 mg V K1 in 20 ml 5%-Glucoselösung langsam intravenös injiziert, in 5 anderen Fällen wurde V K2 verwandt.
Das Blut wurde vor der K2-Injektion und 12 Std nach Austauschtransfusion für den TEG-Test entnommen.
Verkürzungsrate von r (Reaktionszeit):
In der K2-Gruppe neigt die Verkürzungsrate von r (in Prozent) zu höheren Werten als in der K1-Gruppe.
Von V K2 wurde daher eine schnellere Wirkung als von V K1 angenommen.
