Résumé

La nature des besoins en soins et les contraintes qui sont appliquées au système de santé imposent de repenser l’offre de soins. Nombre de pays confrontés au même constat ont introduit le concept de pratique avancée en soins infirmiers et obtiennent des résultats cliniques satisfaisants. Le projet de loi de santé actuellement en discussion prévoit dans son chapitre « innovation » d’instaurer la pratique avancée pour les infirmières. Dans ce contexte, il est nécessaire de clarifier ce concept à l’aide des cadres de référence internationaux validés. C’est une étape préliminaire à la déclinaison de cette fonction en France. La pratique avancée en soins infirmiers est résolument centrée sur la pratique clinique et repose avant tout sur une solide expertise infirmière. L’infirmière de pratique avancée (IPA) travaille en collaboration avec l’équipe pluriprofessionnelle. Le leadership qui caractérise l’IPA l’amène à adopter une approche systémique et à promouvoir l’innovation dans les milieux de soins. La contribution possible d’une IPA dans un service de réanimation sera envisagée, afin de mieux comprendre la nature et l’intérêt de ce nouveau métier infirmier.

Abstract

The supply of care should be reorganized because of health care needs and the constraints that are applied to the health system. Number of countries facing the same conclusion introduced the concept of advanced practice nursing and obtained satisfactory clinical outcomes. The health law currently under discussion in French parliament propose to establish advanced practice for nurses. In this context, it is necessary to clarify the concept of advanced practice nursing using validated international frameworks. This is a preliminary step to the introduction of this function in France. Advanced practice nursing ismainly focused on clinical practice and is based primarily on a strong nursing expertise. The advanced practice nurse (APN) is working with the multiprofessional team. The leadership that characterizes the APN leads to a systemic approach, and to promote innovation in health care environments. The possible contribution of APN in an intensive care unit will be considered in order to understand better the nature and the interest of this new nursing role.
