Open reconstruction of large tears of the rotator cuff

Abstract

There is no standardized treatment for large and massive tears of the rotator cuff. Due to non-uniformity in patients and a variety of biological aspects, standardization is difficult. Operative repair of large tears of the rotator cuff can be technically challenging because of retraction and inelasticity of the tendons, bursal scarring, muscle atrophy and fatty degeneration. Various solutions for the treatment of massive rotator cuff tears are available such as debridement, partial reconstruction, bicepstenoplasty or muscle transfer. Satisfactory clinical results have been reported both arthroscopically and open. Defects of the subscapularis tendon imply the only medical indication for open reconstruction. Defects on the superior and posterosuperior part of the rotator cuff can be reconstructed either arthroscopically or open, depending on the experience and the operative skills of the surgeon. The effectiveness of augmentation with patches is not determined. Autologous growth hormones contained in platelet rich plasma may have a positive influence on tendon healing.

Zusammenfassung

Die Behandlung von großen Rotatorenmanschettenrupturen ist nicht standardisiert. Dies liegt einerseits an dem uneinheitlichen Patientengut, andererseits an der biologischen Varianz der Sehnendefekte. Im Zeitalter zunehmender arthroskopischer Verfahren werden vor allem an schulterchirurgischen Zentren kleinere Defekte nicht mehr offen rekonstruiert. Die Untersuchungen mehrerer Zentren der letzten Jahre zeigen, dass sowohl arthroskopische als auch offene Rekonstruktionen zufriedenstellende Ergebnisse erzielen. Die Entscheidung zur arthroskopischen bzw. offenen Rekonstruktion der superioren und posterosuperioren Defekte wird anhand der Fertigkeit und der Erfahrung des jeweiligen Operateurs gefällt. Alleinige Indikation zur offenen Vorgehensweise besteht bei zu rekonstruierenden Läsionen der Subskapularissehne. Bei primär nicht zu rekonstruierenden Läsionen haben sich in den letzten Jahren bei jüngeren Patienten offene Muskeltransferoperationen durchgesetzt. In den nächsten Jahren könnten bisher noch nicht ausreichend untersuchte Verfahren wie zum Beispiel der Einsatz von Kollagen-Interponaten als Lückenschließer oder auch autologe Wachstumsfaktoren (PRP – Platelet Rich Plasma) die Sehnenheilung bei der Rekonstruktion großer Rotatorenmanschettendefekte positiv beeinflussen.
