Hintergrund
Tuberkulose (TB) zählt unverändert zu den wichtigsten Infektionskrankheiten weltweit. Nach den Schätzungen der Weltgesundheitsorganisation (WHO) erkrankten im Jahr 2011 8,7 Mio. Menschen neu an Tuberkulose, 1,4 Mio. verstarben daran.
Fragestellung
Dieser Artikel gibt einen Überblick über das aktuelle Vorkommen von Tuberkulose weltweit, in Europa und in Deutschland. Dabei wird das Auftreten von Resistenzen sowie die Koepidemie aus TB und HIV/AIDS besonders berücksichtigt, da sie die epidemiologischen Entwicklungen maßgeblich beeinflussen.
Material und Methode
Es finden internationale und nationale Berichterstattung sowie ausgewählte epidemiologische Studien Berücksichtigung. Die Herausforderungen für die Tuberkulosekontrolle in Deutschland werden diskutiert.
Ergebnisse
Global ist ein leichter Rückgang der Tuberkuloseinzidenzen zu verzeichnen. Dies wird unter anderem auf eine verbesserte diagnostische und therapeutische Versorgung, auch HIV-infizierter Patienten, zurückgeführt. Besorgniserregend sind jedoch die zum Teil erheblichen Resistenzraten, vor allem in einigen Staaten der ehemaligen Sowjetunion. In der Europäischen Union und im Europäischen Wirtschaftsraum (EU/EWR) sind die Tuberkulosefallzahlen und -inzidenzen vergleichsweise niedrig, doch auch hier verursacht die TB eine erhebliche Krankheitslast. In Deutschland werden – wie in einigen anderen westeuropäischen Niedriginzidenzländern – stagnierende Fallzahlen sowie eine leichte Zunahme der Kindertuberkulosen beobachtet. Daher dürfen auch hier die Bemühungen um eine erfolgreiche Tuberkulosekontrolle keinesfalls nachlassen.
Abstract
Background
Tuberculosis remains one of the most important infectious diseases worldwide. For 2011 the World Health Organization estimated that there were 8.7 million new cases of tuberculosis and 1.4 million deaths from tuberculosis.
Objectives
This article gives an overview on the current tuberculosis (TB) situation worldwide, in Europe as well as in Germany. Special attention is given to drug resistance and the HIV and tuberculosis coepidemic as they strongly impact on the epidemiology of tuberculosis.
Methods
International and national epidemiological reports on tuberculosis as well as selected epidemiological studies were considered. Challenges for tuberculosis control in Germany are discussed.
Results
Globally, a slight decline in tuberculosis incidence is currently being observed. This is partly attributed to improvements in diagnostic and therapeutic care and also for HIV infected patients. However, drug resistance rates are causing concern, especially in several newly independent states of the former Soviet Union. Although tuberculosis case numbers and incidences in the European Union and the European Economic Area (EU/EEA) are comparably low, tuberculosis still represents a considerable burden of disease. In Germany, as in several other western European countries with a low incidence of tuberculosis, case numbers are stagnating and childhood tuberculosis is slightly increasing. Therefore, efforts to ensure an effective tuberculosis control cannot be allowed to wane.

Multidrug resistant tuberculosis (MDR-TB): Resistenz gegenüber mindestens den beiden wichtigsten Antituberkulotika Isoniazid und Rifampicin.
Extensively drug resistant tuberculosis (XDR-TB): MDR-TB mit zusätzlicher Resistenz gegenüber einem der Fluorchinolone und mindestens gegenüber einem der injizierbaren Medikamente Amikacin, Kanamycin oder Capreomycin.
