Zusammenfassung
Die Elektrokrampftherapie (EKT) als Therapieoption erlebt derzeit eine Renaissance, zum Teil hat sie den Status einer Therapie der Wahl erreicht, wie z. B. bei wahnhaften Depressionen und anderen depressiven Erkrankungen mit schwerer Symptomatik. Nach einem Therapieerfolg durch EKT wird jedoch gewöhnlich eine medikamentöse Erhaltungstherapie begonnen. Die Durchführung einer Erhaltungs-EKT und anschließender prophylaktischer EKT wird in allgemeinen Richtlinien bisher nicht oder nur am Rande erwähnt. In diesem Artikel sollen Richtlinien für die Erhaltungs-EKT aufgezeigt werden, die sich aus den gegenwärtigen Therapieempfehlungen und der aktuellen Studienlage ergeben. Insbesondere zur Indikation, zur Durchführung, zu Komedikation und Komorbidität sowie zu Nebenwirkungen und Kosten wird Stellung genommen. Zusammenfassend wird festgestellt, dass in Deutschland die Indikation zur Erhaltungs-EKT aufgrund verschiedener vermeintlicher Probleme, Stigmatisierungen und Ängste viel zu selten gestellt wird. Dementsprechend wird eine weitere Verbreitung der Erhaltungs-EKT empfohlen.
Summary
Nowadays, electroconvulsive therapy (ECT) is undergoing a renaissance in psychiatry: it is now considered a first-line therapy for treating psychotic depression or other disorders with severe depressive symptoms. Surprisingly, ECT is most commonly not used as continuation therapy after acute remission. With rare exceptions, antidepressive medication is chosen for this purpose. The use of continuation ECT (cECT) and subsequent maintenance ECT (mECT) is not or just marginally mentioned in practice guidelines. In this article, we suggest guidelines for cECT, taking therapy recommendations and recent studies into account. Particularly, indication, management, comedication and comorbidity, side effects, and costs are examined. Today, cECT is underindicated as a result of assumed problems, fears, and stigmas. We would therefore recommend broader use of this proven treatment tool for keeping major depression in remission.
