Zusammenfassung
Hintergrund. Kinder mit frühzeitiger oraler Infektion durch Streptococcus mutans entwickeln in höherem Maß Karies als nicht oder später infizierte Kinder. Die Primärinfektion der Mundhöhle mit diesem Keim findet erst nach dem Durchbruch der ersten Milchzähne statt, wobei eine orale Besiedelung auch noch zu einem späteren Zeitpunkt erfolgen kann.
Infektion. Wird davon ausgegangen, dass überwiegend Mütter ihre Kinder mit Streptococcus mutans infizieren, kommen in erster Linie als Übertragungswege von der Mutter abgeleckte Löffel, Nuckel oder auch Sauger einer Milchflasche in Frage. Demzufolge kann die Infektion der kindlichen Mundhöhle mit Streptococcus mutans zuerst durch Verzicht auf Speichelkontakt, aber auch durch die Reduktion der oralen Streptokokkenbesiedelung der Betreuungspersonen vermieden oder die Zahl der übertragenen Keime definitiv verringert werden.
Vorsorgeuntersuchungen. Dies setzt die frühzeitige Erkennung des Kariesrisikos insbesondere von schwangeren Müttern durch geeignete mikrobiologische Speicheltests im Rahmen von Vorsorgeuntersuchungen voraus.
Abstract
Background. The children with early oral infection of mutans streptococci are developing caries to a higher degree compared to not or later infected children. The primary oral infection with that germ only occurs after the eruption of the first primary teeth but the oral colonization just could appear at a later time too.
Infection. With regard to the assumption that the mothers mainly infect their children with mutans streptococci, so the spoons, the pacifiers or the nipples from baby bottles licked off first are being possible for the transmission. Therefore the risk of an oral infection with mutans streptococci in children could be diminished by avoiding the contact to the saliva and also by reducing the oral colonization with mutans streptococci of the child minder as well as the number of the germ transmitted.
Medical checkup. It requires the early detection of the caries risk especially in pregnant women using suitable microbiological saliva-test methods within the medical checkup.
