Résumé
À partir de l’étude EUROFORS (European Study of Forsteo) qui a démontré l’efficacité du tériparatide (analogue de la PTH) sur les densités minérales osseuses (DMO) chez des femmes ayant une ostéoporose fracturaire postménopausique, cette étude en sous-groupe analyse ses effets en seconde ligne de traitement antiostéoporotique. Deux cent quarante-cinq femmes ont été suivies et traitées par tériparatide durant deux ans. Les valeurs des DMO ainsi que les marqueurs de formation osseuse ont été analysés en fonction du traitement prescrit dans le passé. Les résultats ont montré avec une augmentation significative des valeurs de DMO et des marqueurs de formation osseuse dès un mois de traitement. L’augmentation au niveau lombaire était constante tandis que l’on observait une déminéralisation initiale au niveau des cols et des hanches. L’étidronate avait un effet plus important, en comparaison des autres bisphosphonates, au niveau vertébral uniquement. Le tériparatide semble donc avoir un effet positif sur les DMO et les marqueurs de formation osseuse au niveau vertébral chez des patientes ayant une ostéoporose postménopausique fracturaire en relais des traitements habituels.
Abstract
The EUROFORS (European Study of Forsteo) study demonstrated the effectiveness of teriparatide (PTH analogue) on bone mineral density (BMD) in post-menopausal women with an osteoporotic fracture. This study follows on to perform a sub-group analysis of the drug as second line anti-osteoporotic treatment. 245 women were followed on teriparatide treatment over two years. BMD measurements and markers of bone synthesis were analysed according to treatment previously received. Results showed a significant increase in BMD values and markers of bone synthesis from one month of treatment. In lumbar BMD the increase was steady, whereas there was initial demineralisation in the neck of femur and the hips. Etidronate was more effective than other bisphosphonates but only in vertebral BMD. Teriparatide appears, therefore, to have a positive effect on vertebral BMD and markers of bone formation in post-menopausal patients with osteoporotic fracture, when it is used to follow on from standard treatments.
