Zusammenfassung
Hintergrund
Chronische Erkrankungen nehmen weltweit zu. Ein Konzept zur Verbesserung der Versorgung chronisch Kranker ist das „Chronic care Modell“ (CCM). Ziel der Studie ist eine Bestandsaufnahme der hausärztlichen Erfahrungen und Ansichten zur Nutzung kommunaler Ressourcen als ein Element des CCM.
Methode
Es wurden Interviews mit 14 Hausärzten durchgeführt. Die Auswertung erfolgt inhaltsanalytisch mit dem Computerprogramm Atlas.ti.
Ergebnisse
Ärzte sehen Ressourcen im Rahmen der Prävention, aber auch Beeinträchtigungen hinsichtlich der Organisation, Koordination und/oder Kooperation. Außerdem werden der hohe zeitliche Aufwand und die geringe Motivation der Patienten, kommunale Angebote zu nutzen, als Barriere gesehen.
Schlussfolgerung
Eine Lösungsmöglichkeit kann ein Roundtable der Akteure auf Gemeindeebene sein. Weiterhin besteht Bedarf an Studien, die sich speziell mit der Relevanz an Gemeindeangeboten innerhalb der biopsychosozialen Versorgung von chronisch Kranken beschäftigen.
Abstract
Background
Chronic diseases are increasing world wide. The chronic care model is a framework to improve the care for chronic ill patients. The goal of this study is to get a survey of the experience and opinions of general practitioners according to the integration of communal resources, one element of CCM.
Method
14 general practitioners were interviewed. Analysis and categorization were carried out with Atlas ti.
Results
General practitioners see resources in prevention but also difficulties in organisation, coordination and/or cooperation. Further on, barriers are the huge amount of time and the lacking motivation of the patients in using communal resources.
Conclusion
One possible solution may be a “roundtable” of the stakeholders in the community. Further studies with certain emphasis on the relevance of community resources within the bio-psycho-social model of health care for patients with chronic conditions are necessary.
