Zusammenfassung

In Deutschland warten etwa ein Drittel aller Paare länger als 1 Jahr auf die Erfüllung ihres Kinderwunsches, ca. 6% bleiben dauerhaft ungewollt kinderlos. Üblicherweise ist der niedergelassene Frauenarzt der erste „Kontaktpartner“ bei Sterilität. Standardisiertes diagnostisches Vorgehen trägt zu effektiven, prognoseadaptierten Behandlungsstrategien bei. Junge Frauen mit ovulatorischen Zyklen, positivem Postkoitaltest und kurzer Kinderwunschdauer profitieren von Methoden der natürlichen Familienplanung. Auch bei ausgeprägten Zyklusstörungen ist je nach Ursache ein Therapieversuch durch den niedergelassenen Gynäkologen gerechtfertigt. Ist nach spätestens 6 Monaten keine Schwangerschaft eingetreten oder ist die Frau über 35 Jahren und bestehen deutlich fertilitätseinschränkende Faktoren wie Tubenpathologie, Oligoasthenoteratozoospermie oder langjähriger Kinderwunsch, sollte eine zügige Überweisung in ein Kinderwunschzentrum erfolgen.

Abstract

In Germany, about one-third of all couples wanting a child have to wait more than 1 year to conceive, and around 6% remain infertile. Normally, infertile couples will first present to a gynaecologist for treatment. A standardised diagnostic approach can lead to efficient treatment strategies adapted to the couple’s prognosis. Young women with ovulatory cycles, a positive postcoital test and short-term infertility may benefit from natural family planning methods. Even in severe cycle disorders, depending on the cause, a therapy attempt by the gynaecologist may be justified. If no pregnancy has been achieved after a maximum of 6 months or if the woman is older than 35 and shows fertility-reducing factors such as tubal pathology, oligoasthenoteratozoospermia or long-term infertility, the couple should be referred to an infertility centre without delay.
