Zeitsparende und effektive Methode der temporären intraoperativen Reposition bei Unterkieferfrakturen

Die Methode der Wahl für die chirurgische Versorgung von Unterkieferfrakturen ist die offene Reposition und Plattenosteosynthese in Verbindung mit maxillo-mandibularer Fixation. Wir berichten über eine zeitsparende und effektive Methode der temporären intraoperativen Reposition bei Unterkieferfrakturen, die zu einer signifikanten Vereinfachung der Operationsprozedur führt. Die einfache und effiziente Technik besteht aus dem Einbringen von 2 FAMI ^® -Schrauben und deren Verbindung mit einer Drahtligatur. Diese wird benutzt in Kombination mit einem monokortikalen Miniplattensystem nach Champy und gestattet den sicheren Verzicht einer maxillo-mandibulären Fixation. Die Resultate bei 78 Patienten mit Unterkieferfrakturen bestätigen die Vorteile und die Verläßlichkeit der temporären Reposition.

Open reduction and rigid fixation with maxillomandibular fixation at least intraoperatively is the method of choice for treatment of mandibular fractures. We report an effective method of internal temporary fixation which significantly facilitates fracture reduction and stabilization. The technique is used in combination with special FAMI screws and a monocortical miniplates system according to Champy, which eliminates in most cases the need for maxillomandibular fixation. Our results in 78 patients with mandibular fractures confirmed the reliability and the efficiency of the internal temporary fixation.
