Messung der Kniegelenkskinästhesie zur Bestimmung von Propriozeptionsdefiziten bei Varusgonarthrose

Zusammenfassung

Bei der Beurteilung der Kniegelenksfunktion bei Varusgonarthrose kommt der Untersuchung von Propriozeptionsveränderungen eine wachsende Bedeutung zu. In Anbetracht der in der Literatur gegenüber den Gelenkstellungssinn-Messungen geäußerten Methodenkritik entwickelten wir ein eigenes Verfahren zur Bestimmung der Kniegelenkskinästhesie als wichtigem Parameter der Propriozeption. Dabei wird bei 5 unterschiedlichen Winkelgeschwindigkeiten der passiven Kniegelenksbewegung die Fähigkeit sowohl zur Bewegungs- als auch zur Stopdetektion bestimmt. Wir untersuchten mit dieser Methode 25 Patienten mit klinisch und radiologisch gesicherter Varusgonarthrose (VGA) sowie 20 altersgleiche Normalpersonen. Neben einer Zunahme der propriozeptiven Fähigkeiten mit steigender Winkelgeschwindigkeit erbrachte die Detektionsfehler-Analyse signifikant höhere Fehlerraten bei der Bewegungs- und Stopdetektion in der VGA-Gruppe gegenüber der Kontroll-Gruppe und somit ein deutliches propriozeptives Defizit bei Gonarthrose. Dagegen zeigten sich bei korrekter Detektion keine signifikanten Unterschiede in den Schwellenwerten der Bewegungs- und Stopdetektion zwischen der VGA- und der Kontroll-Gruppe. Anhand der vorliegenden Kinästhesiemessungen konnten wir ein deutliches propriozeptives Defizit bei Varusgonarthrose zeigen. Das Fehlen von Unterschieden in den Detektionsschwellenwerten zwischen Kontroll- und VGA-Gruppe einerseits und die deutlich erhöhten Detektionsfehlerraten der VGA-Patienten andererseits betrachten wir als Hinweis darauf, daß das propriozeptive Defizit bei Varusgonarthrose maßgeblich in einer Störung der Verarbeitung propriozeptiver Afferenzen seinen Ursprung hat.

Summary

Studying knee osteoarthrosis proprioceptive deficits play a growing role. However, the methods for the detection of these deficits using joint position sense measurements have been criticized recently. Therefore, we developed a new method for the measurement of knee-joint kinaesthesia. At 5 different angular velocities of passive knee movement both movement and stop detection were tested. Tests were performed both in 25 patients with osteoarthrosis of the knee and in a control group of 20 age related normal subjects. In addition to the measurements of movement detection and stop-detection thresholds, a specific detection-failure analysis was performed in our study. In addition to a significant improvement of proprioceptive abilities with increasing angular velocities, detection failure analysis brought about significant differences between patients with knee osteoarthrosis and normal subjects yielding a significant proprioceptive deficit in osteoarthrosis of the knee. In contrast, in case of correct movement detection or stop-detection there were no significant differences in the detection-threshold values between normal and ostheoarthrotic knees. In conclusion, both the lack of differences in detection thresholds between the arthrosis and the control-groups and the increased detection failure rates in arthrosis patients gave evidence for these proprioceptive differences being a result of central nervous dysregulation in knee osteoarthrosis.
