Zusammenfassung
Die adjuvante Systemtherapie ist eine der wesentlichen Säulen in der Behandlung des Mammakarzinoms und umfasst heute neben der klassischen Chemotherapie die endokrine und die Antikörpertherapie. Entscheidend ist die korrekte leitlininiengerechte Indikationsstellung, um der einzelnen Patientin eine optimale Behandlung mit maximalen Nutzen und möglichst geringen Nebenwirkungen anbieten zu können. Hierzu ist eine individuelle Risikoeinschätzung notwendig, sie beruht auf klassischen Prognosefaktoren und auch auf etablierten Biomarkern, wie Hormon- und Her2/neu,Rezeptorstatus. Zudem stellt der Menopausenstatus ein weiteres wichtiges Kriterium in der Therapieplanung dar, da sich die endokrinen Behandlungsoptionen in den letzen Jahren maßgeblich erweitert haben. Aktuell werden Patientinnen mit einem primären Mammakarzinom hinsichtlich des Rezidivrisiko in 3 Gruppen eingeteilt: niedriges, mittleres und hohes Risiko für die Entwicklung von Rezidiven; danach ergeben sich je nach Rezeptorstatus unterschiedliche Therapieempfehlungen und Medikamentenkombinationen. Abstract
Adjuvant systemic therapy is a main element in the treatment strategy for early breast cancer, including classic chemotherapy with cytotoxic drugs and endocrine- and antibody-mediated methods. It is very important to individualize treatment decisions for each single patient based on current evidence-based guidelines to achieve maximum treatment benefit and minimal side effects. Treatment decisions are based on individual risk assessment which includes the evaluation of classic prognostic factors and established biological marker receptor status. Menopause status is also pertinent to choice of treatment, since new endocrine therapeutic options are available for certain patient subgroups. Women with primary breast cancer are categorized according to low, intermediate, or high risk of local or distant recurrence, and depending on receptor status, different treatment strategies with distinct drug combinations can be offered.
