Zusammenfassung
Hintergrund
Botulinum-Neurotoxin A (BoNTA) ist seit 1993 zur Behandlung des rotatorischen Torticollis spasmodicus (TS), des Blepharospasmus (BLSP) und koexistenter Hyperkinesen sowie des Hemispasmus facialis (HSF) in Deutschland zugelassen.
Methodik
Wir beschreiben die Langzeiterfahrung an 130 Patienten, die mindestens 6 Jahre, maximal bis 16 Jahre mit BoNTA in diesen Indikationen behandelt wurden.
Ergebnisse
Im Untersuchungszeitraum 2000 bis 2006 nahmen die mittleren Dosen, die für einen befriedigenden Therapieeffekt erforderlich waren, beim BLSP und HSF nur mäßig und in nicht signifikantem Unfang zu, während sich die mittleren Dosen beim TS deutlich und signifikant erhöhten. Die Therapieintervalle blieben während dieses Zeitraums unverändert. Sie waren beim Hemispasmus facialis etwas länger als bei den beiden anderen Indikationen. Unerwünschte Effekte wurden mit einer Häufigkeit zwischen 2,9% (TS) und 4,1% (HSF) beobachtet. Sie traten mit zunehmender Behandlungsdauer seltener auf. Ein Therapieabbruch erfolgte jenseits eines Behandlungszeitraums von 5 Jahren nur durch den Tod der Patienten. Wir beobachteten in dieser Gruppe keine sekundären Versager aufgrund einer Antikörperbildung.
Schlussfolgerung
Fokale Dystonien können über viele Jahre effektiv mit BoNTA behandelt werden, wobei eine Kontinuität des Behandlers eine optimale Wahl von Dosis und Injektionsabstand begünstigt.
Summary
Background
Although botulinum neurotoxin A has been used for many years to treat focal dystonia-like blepharospasm (BLSP), cervical dystonia (CD), and hemifacial spasm (HFS) as vascular compression disorder of the facial nerve, there are few data on its long-term efficacy.
Methods
We analysed dosages and injection intervals in a group of 130 patients with various focal dystonias treated for 6–16 years.
Results
During the observation period from 2000 to 2006, the mean dosage required to obtain appropriate clinical benefit increased only slightly in BLSP and HSF. In CD however, significantly higher doses were necessary for good clinical results. The mean intervals between injection varied highly between individual patients but did not change significantly between 2000 and 2006. Adverse effects, mostly minor, were reported in 4.2% of all treatment cycles in HSF; the frequencies were 3.8 for BLSP and 2.9 for CD, respectively. None of the patients stopped treatment because of resistance due to antibodies.
Conclusion
Botulinum neurotoxin A is effective in long-term treatment of focal dystonia when injection intervals and dosages are chosen carefully.
