Zusammenfassung
Spezifische Immuntherapie
Die spezifische Immuntherapie (SIT, Hyposensibilisierung) gilt zurzeit als einzige Behandlung allergischer Erkrankungen, die sowohl die Symptomlast verringert als auch Einfluss auf das Fortschreiten dieser Krankheiten hat.
Indikationen
Gesicherte Indikationen für eine SIT bei Kindern sind v. a. die allergische Rhinitis/Rhinokonjunktivitis, Asthma bronchiale und die Insektengiftallergie mit systemischen Reaktionen. Diskutiert wird ihr Einsatz bei Patienten mit atopischer Dermatitis bzw. mit oralem Allergiesyndrom. Voraussetzung sind der Nachweis einer IgE-vermittelten Sensibilisierung (durch Analyse spezifischer IgE-Antikörper oder Hautpricktest), ein eindeutiger Zusammenhang zwischen Sensibilisierung und klinischen Symptomen, die Verfügbarkeit untersuchter Allergenextrakte mit einem Wirksamkeitsnachweis für die jeweilige Erkrankung und die Unmöglichkeit bzw. die fehlende Effektivität einer Allergenkarenz.
Kontraindikationen
Als solche gelten bei Kindern ein nur teilweise oder unkontrolliertes Asthma bronchiale [FEV1 < 70 % (FEV1: forciertes exspiriertes Volumen nach 1 s)], schwere akute Autoimmunerkrankungen, Immundefizienz und akute Inflammationssyndrome, akute maligne Erkrankungen, Therapie mit β-Blockern und ACE-Hemmern (ACE: „angiotensin-converting enzyme“) sowie kardiovaskuläre Erkrankungen mit konsekutivem Risiko bei Epinephringabe.
Abstract
Allergen-specific immunotherapy
Allergen-specific immunotherapy (SIT: specific immunotherapy, hyposensizitation) represents at present the single therapeutic entity for allergic disease, which reduces symptom burden and demonstrates disease-modifying effects.
Indications
In children with allergic rhinitis/rhinoconjunctivitis, allergic asthma, and systemic reactions to hymenoptera stings, SIT has proven efficacy. The use of SIT in children with atopic dermatitis and oral allergy syndrome is under discussion. Prerequisites for the initiation of SIT are the detection of IgE (immunoglobulin E) antibodies or evidence of sensitization with the skin prick test to clinically relevant allergens, the availability of allergen extracts with proven efficacy for the underlying allergic disease, and the impossibility of allergen avoidance.
Contraindications
Contraindications for SIT are partially or fully uncontrolled asthma [FEV1 < 70 % (FEV1: forced expiratory volume in 1 s)], severe acute autoimmune disorders, severe immunodeficencies, acute inflammatory syndromes, malignancies, treatment with β-blockers and ACE (angiotensin-converting enzyme) inhibitors and cardiovascular diseases with increased risk of adverse events during administration of epinephrine.
