Zusammenfassung
Die Gliedmaßenapraxie ist eine Störung der motorischen Handlungsplanung und -ausführung, meist bedingt durch eine Läsion des linken frontoparietotemporalen Kortex, oft aber auch durch Läsionen der tiefen weißen Substanz, von Leitungsbahnen oder des Thalamus. Die Gliedmaßenapraxie teilt sich ein in die ideatorische und die ideomotorische Apraxie, wobei unter der ideatorischen Apraxie ein Fehler bei Handlungen mit Objektgebrauch besteht (Kaffeekochen), bei der ideomotorischen Apraxie ein Fehler bei Handlungen ohne Objektgebrauch (Ausführen von Gesten, Pantomime, Imitation von Bewegungen). Weiters wird auf die Testungsmöglichkeiten der Apraxien eingegangen. Die ideatorische Apraxie kann durch Überprüfung von Handlungen mit Objektgebrauch untersucht werden, zur Testung der ideomotorischen Apraxie eignet sich eine gezielte Überprüfung von Pantomime, Gesten und Imitation. Zuletzt wird noch ein Überblick über die Behandlungsmöglichkeiten bei Gliedmaßenapraxie gegeben.
Abstract
Apraxia is the disturbance of planning and of execution of motor activity. It is not caused by a lesion or a disturbance of the motor or sensory nervous system, it is elicited by a dysfunction of an area in the left cortex of the brain. This area in the left frontoparietotemporal hemisphere is located right beside the area for speech. Therefore it is not unusual that patients with apraxia suffer from aphasia as well. The two different types of limb apraxia are ideomotor apraxia and ideational apraxia. Ideomotor apraxia is apraxia without tool use, it includes imitation of positions of hands and fingers, performance of gestures on demand, and pantomime of object use. Ideational apraxia is apraxia with tool use like cutting with a knife or utilizing a pencil.
