Summary

The field covered by this review is expanding with great rapidity. It now presents a remarkable and in some ways confusing mélange of observations, derived from experiments using a wide range of techniques, on various species of animal, and from investigations in man in health and disease. Out of this fertile muddle, intensely stimulating to research, useful hypotheses are beginning to emerge, and provide a tentative physiological framework to which clinical observations may be related. It is essential to appreciate, however, that in a growing subject of such complexity, valid generalisations are not easy to make, and the temptation to impose order prematurely must be resisted.
