Zur Substitutionsbehandlung beim kongenitalen Faktor V-Mangel

Zusammenfassung

Es wird über die prophylaktische Substitutionsbehandlung zur Durchführung von Zahnextraktionen bei einer Patientin mit hochgradigem kongenitalem Faktor-V-Mangel (1%) berichtet. Vor dem Eingriff wurde eine Blutwechseltransfusion durchgeführt. Anschließend erhielt die Patientin über drei Tage Tiefkühlplasma. Die Extraktion von 8 Zähnen und Wurzelresten des Oberkiefers verlief ohne Nachblutungen. Die biologische Halbwertszeit des Faktors V beträgt nach den durchgeführten Untersuchungen etwa 12 Stunden. Fortlaufende Bestimmungen des Faktors-V-Plasmaspiegels bei einem der Spender nach der Blutwechseltransfusion ergaben einen weit über den Ausganswert hinausgehenden Anstieg dieses Gerinnungsfaktors.

Summary

The authors report on the prophylactic substitution treatment for the performance of dental extractions in a female patient with severe congenital factor-V-deficiency (1%). An exchange blood transfusion was performed prior to the procedure. Subsequent to this the patient received refrigerated plasma for 3 days. Extraction of 8 teeth and dental root remnants from the maxilla was performed and postoperative bleeding did not occur. According to the performed tests the biological half-life of factor V is approximately 12 hours. Serial follow-up determinations of the factor-V-plasma level after the exchange blood transfusion in one of the donors showed an elevation of this coagulation factor which was far in excess of the initial value.
